摘 要:目的:探究节段分析法产前超声诊断胎儿心脏畸形在基层医院的应用价值。方法:选取2019年6月至2023年5月在我院进行产前超声筛查的孕妇5760例,采用节段分析法进行产前超声诊断胎儿心脏畸形,根据产后新生儿超声心动图结果或解剖结果评估节段分析法产前超声诊断的诊断价值。结果:5760例胎儿中,采用节段分析法产前超声诊断出心脏畸形胎儿71例,检出率1.23%,其中永存左上腔静脉检出率最高(19例,26.76%);5760例胎儿中,病理诊断出心脏畸形胎儿66例,实际检出率1.14%,其中永存左上腔静脉检出率最高(19例,28.78%);与病理诊断结果比较,节段分析法产前超声诊断胎儿心脏畸形的灵敏度为96.96%,特异度为99.88%,准确率为99.84%,两种方法诊断一致性较高(Kappa=0.934)。结论:节段分析法产前超声诊断胎儿心脏畸形操作简单,准确率较高,在基层医院推广应用,能促进当前基层胎儿先天性心脏病筛查的普及。Objective:To explore the application value of segmental analysis in prenatal ultrasound diagnosis of fetal cardiac malformation in primary hospitals.Methods:5,760 pregnant women who underwent prenatal ultrasound screening in our hospital from June 2019 to May 2023 were selected,and segmental analysis was used to diagnose the fetal cardiac malformations by prenatal ultrasound diagnosis.The diagnostic value of segmental analysis in prenatal ultrasound diagnosis was evaluated according to the postpartum and neonatal echocardiography results or anatomical results.Results:Among 5,760 fetuses,71 cases were diagnosed with cardiac malformation by segmental analysis in prenatal ultrasound diagnosis,with the detection rate of 1.23%,among which persistent left superior vena cava had the highest detection rate(19 cases,26.76%).Among 5,760 fetuses,66 cases were pathologically diagnosed with cardiac malformation,and the actual detection rate of was 1.14%,among which the highest rate was found in permanent left superior vena cava(19 cases,28.78%).Compared with pathological diagnosis result,the sensitivity,specificity and accuracy rate of segmental analysis in prenatal ultrasound diagnosis were96.96%,99.88%and 99.84%in the diagnosis of fetal cardiac malformation,indicating a high diagnostic consistency between the two methods(Kappa=0.934).Conclusion:Segmental analysis in prenatal ultrasound diagnosis of fetal cardiac malformation has simple operation and high accuracy rate,and it can promote the popularization of screening for congenital heart disease in primary hospitals.
