摘 要:目的探讨粒细胞巨噬细胞集落刺激因子(GM-CSF)、神经生长因子(NGF)、白介素-17(IL-17)在实验性自身免疫性前列腺炎(EAP)大鼠前列腺组织中表达的意义。方法建立实验性自身免疫性前列腺炎大鼠模型并分为对照组、EAP组、抗GM-CSF对照组(阻断对照组)、抗GM-CSFEAP组(阻断EAP组),予痛觉行为测试。取各组大鼠前列腺组织,免疫组化观察GM-CSF在前列腺组织中表达;通过逆转录聚合酶链反应(RT-PCR)和Western blot方法检测GM-CSF、NGF、IL-17的mRNA及蛋白表达。结果痛觉测试:抗GM-CSF组在EAP中显示出比EAP组更轻的慢性盆腔疼痛。HE染色:抗GM-CSF组在EAP诱导后组织炎症反应较少。对照组EAP炎症重于抗GM-CSF组。免疫组化:EAP组大鼠前列腺组织在免疫组化检测中,GM-CSF呈阳性表达(主要在细胞核表达)。RT-PCR:抗GM-CSF组EAP后50 d用RT-PCR检测IL-17和NGF,抗GM-CSF显著降低了EAP诱导的前列腺组织中IL-17和NGFmRNA表达的增加。Western blot:抗GM-CSF组EAP后50 d用WB检测IL-17和NGF蛋白表达量,抗GM-CSF显著降低了EAP诱导的前列腺组织中IL-17和NGF蛋白的表达。结论GM-CSF、NGF、IL-17在EAP大鼠前列腺组织中表达增加,它们可能是慢性前列腺炎/慢性盆腔疼痛综合征(CP/CPPS)的重要炎症介质;且在抗GM-CSF后NGF、IL-17的表达下降,说明GM-CSF很有可能成为CP/CPPS的潜在治疗靶点。Objective To investigate the significance of granulocyte macrophage colony-stimulating factor(GM-CSF),nerve growth factor(NGF)and interleukin-17(IL-17)in the prostate tissue of rats with experimental autoimmune prostatitis(EAP).Methods EAP rat models were established and divided into control group,EAP group,anti-GM-CSF group(blocking control group)and anti-GM-CSFEAP group(blocking EAP group).Pain behaviors were tested.The pathological changes were observed with HE staining.The mRNA and protein expressions of GM-CSF,NGF and IL-17 were detected with RT-PCR and Western blot.Results Pain test showed the anti-GM-CSF group had less chronic pelvic pain than the EAP group.HE staining showed the anti-GM-CSF group had less tissue inflammatory response.The EAP inflammation score was higher in the control group than in the anti-GM-CSF group.Immunohistochemistry showed GM-CSF was positive in the EAP group(mainly in the nucleus).RT-PCR and Western blot results showed the mRNA and protein expressions of IL-17 and NGF significantly decreased 50 days after EAP in the anti-GM-CSF group.Conclusion Increased expressions of GM-CSF,NGF and IL-17 in prostate tissue of EAP rats may be important inflammatory mediators of chronic prostatitis/chronic pelvic pain syndrome(CP/CPPS);decreased expressions of NGF and IL-17 after resistance against GM-CSF indicate that GM-CSF may be a potential therapeutic target for CP/CPPS.
