中国人鼻背纹的分型和治疗策略探索

摘　　要：目的 观察中国人群的鼻背纹分型并探讨相应的精准化治疗策略。方法 回顾性分析自2021年7月至2022年9月,于中国医学科学院北京协和医院整形美容外科治疗鼻背纹的238例患者,女性201例,男性37例。术前在标准拍摄条件下拍摄皱鼻照片,进行鼻背纹分型。选择6个常规注射点注射A型肉毒毒素,根据个体的分型调整注射点的注射剂量。术后3~5周进行随访,采用面部皱纹评分评估治疗效果以及有无并发症发生。结果 纳入患者中复合型88例(37.0%),横纹型70例(29.4%),斜纹型52例(21.8%),竖纹型28例(11.8%)。面部皱纹评分:术前为(2.50±0.50)分,术后为(1.03±0.54)分,治疗前后评分差异具有统计学意义(P<0.05)。术后14例患者出现了淤青,9例患者出现微笑时唇部的轻微不对称,其他患者无其他严重并发症发生。结论 中国人的鼻背纹可分为复合型、横纹型、斜纹型以及竖纹型。精准化鼻背纹治疗策略具有良好的临床效果。Objective To observe the classification of nasal dorsal lines in Chinese population and explore the corresponding precise treatment strategy.Methods From July 2021 to September 2022,238 patients(201 female and 37 male)receiving treatments for nasal dorsal lines in the Department of Plastic and Reconstructive Surgery of Peking Union Medical College Hospital were enrolled.Before the injection,photographs were captured at rest and at grimace under the standard conditions,then the nasal dorsal lines were classified.Six regular injection points were selected.The injection dose at each point was adjusted according to the specific wrinkle type.Follow-up was performed at 3 to 5 weeks after injection.The treatment effect was evaluated by Facial Wrinkle Scale.Complications were also recorded.Results Among the patients included,88 cases were complex type(37.0%),70 cases were transverse type(29.4%),52 cases were oblique type(21.8%),and 28 cases were vertical type(11.8%).The average preoperative wrinkle score was(2.50±0.50),and the postoperative wrinkle score was(1.03±0.54),with a significant difference(P<0.05).Fourteen patients had bruising and 9 patients had slight asymmetry of lips when smiling.No other serious complications were recorded.Conclusion The nasal dorsal lines of Chinese people can be categorized into complex,transverse,oblique,and vertical types.The precise treatment strategy adopted in this paper has good clinical effect.
