等离子光纤清除法与小切口修剪法治疗腋臭的疗效比较

摘　　要：目的 比较等离子光纤清除法与小切口修剪法治疗腋臭的疗效及安全性。方法 回顾性分析自2019年1月至2021年1月,武汉市第一医院整形外科门诊收治的89例腋臭患者的临床资料。按治疗方法分为两组,46例采用等离子光纤清除法治疗,为光纤组;43例采用小切口修剪法治疗,为小切口组。所有患者均进行随访观察,记录并发症发生情况。根据疗效、皮肤病生活质量指数(dermatology life quality index, DLQI)评分以及患者满意度对腋臭治疗效果及安全性进行评估。结果 89例患者获随访12~24个月。光纤组与小切口组间的治愈率差异无统计学意义(P>0.05)。术后1个月,光纤组满意率明显高于小切口组,差异有统计学意义(P<0.05);术后3个月和6个月,两组满意率差异均无统计学意义(P>0.05)。两组治疗后DLQI评分均显著下降,差异有统计学意义(P<0.05)。小切口组2例患者术后出现血肿,2例出现创面延迟愈合;光纤组1例出现皮肤灼伤、创面延迟愈合,两组间总并发症发生率差异无统计学意义(P>0.05)。结论 等离子光纤清除法与小切口修剪法均为安全有效的腋臭治疗方法,具有恢复期较短、满意率较高等优势。Objective To compare the efficacy and safety of plasma fiber removal method and small incision subcutaneous pruning method in the treatment of axillary osmidrosis.Methods The clinical data of axillary patients admitted in the Department of Plastic Surgery of Wuhan First Hospital from January 2019 to January 2021 were retrospectively analyzed.A total of 46 patients were treated by plasma fiber removal method and set as the optical fiber group;43 patients were treated by small incision subcutaneous pruning method and set in the small incision group.All patients were followed up to record the complications.The efficacy and safety of axillary treatment were assessed by efficacy,dermatological quality of life index(DLQI)score and patient satisfaction.Results All patients were followed up for 12 to 24 months.The cure rate was not significantly different between the optical fiber and small incision groups,with no statistical difference(P>0.05).One month after surgery,the satisfaction survey of the two groups was significantly higher in the optical fiber group than the small incision group(P<0.05),no significant difference in satisfaction rates between the two groups at 3 and 6 months after surgery(P>0.05).DLQI scores decreased significantly after surgery and were statistically different(P<0.05).Two patients in the small incision group had postoperative hematoma,2 patients had delayed wound healing,and 1 patient in the optical fiber group had skin burns and delayed wound healing.The total complication rate was no significantly different between the two groups,and no statistically significant(P>0.05).Conclusion The plasma fiber removal method and the small incision pruning method are both safe and effective treatment methods for bromhidrosis,with the advantages of short recovery period and high satisfaction rate.
