摘　　要：目的 系统评价富血小板血浆(platelet-rich plasma, PRP)联合自体脂肪移植在面部年轻化治疗的有效性,为临床治疗提供参考依据。方法 检索Pubmed、EMbase、Cochrane Library、Web of science、中国知网、万方、维普等中英文数据库,从建库至2021年12月已发表的关于PRP联合自体脂肪移植在面部年轻化应用的相关研究,观察组为PRP联合自体脂肪移植,对照组为单纯自体脂肪移植。由2名评价员独立筛选文献、提取资料,并对文献进行质量评估,随机对照研究采用Cochrane偏倚风险评估工具,非随机对照研究采用纽卡斯尔-渥太华量表(newcastle-ottawa scale, NOS)进行文献质量评估,采用Rev Man 5.3软件对文献进行Meta分析。结果 共纳入14篇文献,包含1013例求美者纳入Meta分析,其中观察组511例,对照组502例。Meta分析结果显示,观察组术后恢复时间、术后并发症发生率较对照组低,而术后自体脂肪移植成活率、治疗有效率、求美者满意率均高于对照组,差异均有统计学意义(P<0.05)。结论 PRP联合自体脂肪移植用于面部年轻化治疗能有效缩短恢复时间、减少术后并发症的发生率、提高自体脂肪移植成活率。该治疗方法安全、有效,值得临床推广应用。Objective To systematically evaluate the effectiveness of platelet-rich plasma combined with autologous fat transplantation for facial rejuvenation,and to provide a basis for clinical treatment.Methods The PubMed,Embase,Cochrane Library,CNKI,Wangfang,CQVIP and other databases were searched.Several studies on platelet-rich plasma combined with autologous fat transplantation for facial rejuvenation from database establishment to December 2021 were collected for meta-analysis.The control group was treated with autologous fat,while the study group was treated with PRP combined with autologous fat.Document screening,quality evaluation and data extraction were conducted independently by the two researchers.Cochrane bias risk assessment tool was used for randomized controlled studies,and newcastle-ottawa scale(NOS)was used for non-randomized controlled studies.RevMan5.3 Meta analysis software was used.Results Fourteen studies included 1013 patients,there were 511 cases in study group and 502 cases in control group.According to meta-analysis,the incidence of postoperative complications and the recovery time after surgery of patients in study group were lower than the control group.While the postoperative fat survival rate,satisfaction of patients and the effective rate of treatment were higher than the control group(P<0.05).Conclusion Platelet-rich plasma combined with autologous fat transplantation can effectively shorten the recovery time,reduce postoperative complications and improve fat survival rate in the treatment of facial rejuvenation.This treatment method is efficacy and safety,it should be considerable for clinical promotion and application.
