血清HE4、CA125联合血浆D-二聚体检测在子宫内膜癌诊断中的应用价值

摘　　要：目的观察血清人附睾蛋白4(human epididymis protein4,HE4)、血清糖抗原125(carbohydrate antigen 125,CA125)联合血浆D-二聚体(D-dimer,D-D)检测在子宫内膜癌诊断中的应用价值。方法选取2021年6月—2023年4月罗定市人民医院收治的恶性子宫内膜病变患者(子宫内膜癌组40例)、子宫良性疾病患者(良性病变组40例)为试验组,以及同期于罗定市人民医院进行体检的健康女性为对照组(健康对照组40例)。统计不同组别及子宫内膜癌不同分期患者的血清CA125、HE4、血浆D-D检测水平。结果子宫内膜癌组血清HE4、CA125、血浆D-D检测更高(P<0.05)。子宫内膜癌组中Ⅰ~Ⅱ期患者血清HE4、CA125、血浆D-D检测水平结果低于Ⅲ~Ⅳ期患者,差异有统计学意义(P<0.05)。子宫内膜癌血清HE4与CA125及D-D呈正相关(r=0.576,0.618);CA125与D-D呈正相关(r=0.649)。联合检测诊断价值更高(P<0.05)。结论子宫内膜癌患者血清HE4、CA125及D-D水平均呈高表达,并与子宫内膜癌患者的临床分期有一定的相关性,可帮助临床判断子宫内膜癌患者的病情进展情况,以三项联合检测具有重要价值。Objective To observe the value of serum human epididymis protein 4(HE4),serum carbohydrate antigen 125(CA125)and plasma D-dimer(D-D)in the diagnosis of endometrial cancer.Methods From June 2021 to April 2023,patients with malignant endometrial diseases(40 cases of endometrial cancer group),patients with benign uterine diseases(40 cases of benign disease group)admitted in Luoding People's Hospital were selected as the test group,and healthy women who underwent physical examination in Luoding People's Hospital at the same time were selected as the control group(40 cases of healthy control group).The levels of serum CA125,HE4 and plasma D-D in patients of different groups and different stages of endometrial cancer were counted.Results The levels of serum HE4,CA125 and plasma D-D were higher in endometrial carcinoma group(P<0.05).The detection levels of serum HE4,CA125,and plasma D-D in patients with stageⅠ-Ⅱendometrial cancer were lower than those in patients with stageⅢ-Ⅳendometrial cancer,and the difference was statistically significant(P<0.05).Serum HE4 levels in endometrial cancer were positively correlated with CA125 and D-D levels(r=0.576,0.618);CA125 was positively correlated with D-D(r=0.649).The diagnostic value of combined testing was higher(P<0.05).Conclusion The serum levels of HE4,CA125,and D-D in patients with endometrial cancer are highly expressed,and there is a certain correlation with the clinical staging of endometrial cancer patients.This can help clinical judgment of the progression of endometrial cancer patients,and the combined detection of the three items has important value.
