摘 要:目的探讨徒手膨肺联合胸廓振动挤压排痰应用于机械通气患者的效果。方法选取惠州市第三人民医院2020年5月—2023年1月收治的行机械通气治疗的患者86例,按照随机数字表法分为两组,每组各43例。对照组采用常规胸部物理治疗,观察组采用徒手膨肺联合胸廓振动挤压排痰治疗。比较两组的恢复情况、肺通气功能、血气指标以及并发症发生情况。结果观察组机械通气时间、呼吸困难消失时间、住院时间短于对照组,总吸痰次数少于对照组,总排痰量高于对照组(P<0.05)。观察组氧合指数(oxygenation index,OI)、动态肺顺应性(dynamic lung compliance,Cdyn)高于对照组,气道阻力(air way resistance,Raw)低于对照组(P<0.05)。观察组氧分压(partial pressure of oxygen,PaO_(2))、血氧饱和度(oxygen saturation,SpO_(2))高于对照组,二氧化碳分压(partial pressure of carbon dioxide,PaCO_(2))低于对照组(P<0.05)。观察组并发症发生率低于对照组(P<0.05)。结论机械通气患者采用徒手膨肺联合胸廓振动挤压排痰,可促进痰液排出,以快速改善患者血气指标和肺通气功能,缩短机械通气时间,降低并发症发生率,加快疾病恢复。Objective To investigate the effect of lung swelling combined with chest vibration squeezing and sputum drainage in patients with mechanical ventilation.Methods A total of 86 patients receiving mechanical ventilation treatment from May 2020 to January 2023 in the Third People's Hospital of Huizhou were divided into two groupsaccording to the random number table method,with 43 cases in each group.The control group was treated with conventional chest physical therapy,and the observation group was treated with freehand lung expansion combined with chest vibration squeezing and sputum drainage.Recovery,lung function,blood gas index and complications were compared between the two groups.Results The mechanical ventilation time,dyspnea disappearance time and hospital stay in the observation group were shorter than those in the control group,the total sputum aspiration times were less than those in the control group,and the total sputum output was higher than those in the control group(P<0.05).The oxygenation index(OI)and dynamic lung compliance(Cdyn)of the observation group were higher than those of the control group,while the airway resistance(Raw)was lower than that of the control group(P<0.05).The partial pressure of oxygen(PaO_(2))and blood oxygen saturation(SpO_(2))in the observation group were higher than those in the control group,while the partial pressure of carbon dioxide(PaCO_(2))was lower than those in the control group(P<0.05).The complication rate of observation group was lower than control group(P<0.05).Conclusion In patients with mechanical ventilation,the use of freehand lung expansion combined with chest vibration extrusion can promote sputum discharge,so as to rapidly improve the blood gas index and lung ventilation function of patients,shorten the time of mechanical ventilation,reduce the incidence of complications,and accelerate the recovery of disease.
