三切口联合入路治疗复杂胫骨平台骨折的临床研究

摘　　要：目的:探讨三切口联合入路治疗复杂胫骨平台骨折的临床效果。方法:选取2021年7月—2022年6月九江市第一人民医院收治的复杂胫骨平台骨折患者70例,根据随机数字表法分成观察组(n=35)和对照组(n=35)。对照组给予单侧钢板内固定术治疗,观察组给予三切口联合入路治疗。比较两组治疗效果、负重时间、住院时间、骨折愈合时间、膝关节功能及术后并发症。结果:观察组(97.14%)治疗优良率高于对照组(77.14%)(P<0.05)。观察组负重、住院、骨折愈合时间较对照组均缩短(P<0.05)。术后3个月,两组美国特种外科医院(HSS)膝关节评分、Lysholm膝关节评分、Rasmussen影像学评分较术前均提高(P<0.05),且观察组均高于对照组(P<0.05)。观察组(8.57%)术后并发症发生率低于对照组(28.57%)(P<0.05)。结论:复杂胫骨平台骨折患者采用三切口联合入路治疗的效果较好,能够缩短患者恢复时间,提高膝关节功能,减少术后并发症发生。Objective:To explore the clinical effect on three incisions combined approach in the treatment of complex tibial plateau fractures.Method:A total of 70 patients with complex tibial plateau fractures admitted to Jiujiang NO.1 People's Hospital from July 2021 to June 2022 were selected,and divided into the observation group(n=35)and the control group(n=35)by the random number table method.The control group was treated with unilateral steel plate internal fixation,and the observation group was treated with three incision combined approach.The treatment effect,weight bearing time,hospitalization time,fracture healing time,knee joint function and postoperative complications of the two groups were compared.Result:The excellent and good rate of the observation group(97.14%)was higher than that of the control group(77.14%)(P<0.05).The weight bearing time,hospitalization time and fracture healing time of the observation group were shorter than those of the control group(P<0.05).Three months after operation,the hospital for special surgery(HSS)knee score,Lysholm knee scale scores and the Rasmussen imaging scores in both groups were improved(P<0.05),and the scores of the observation group were higher than those of the control group(P<0.05).The incidence of postoperative complications of the observation group(8.57%)was lower than that of the control group(28.57%)(P<0.05).Conclusion:The treatment of complex tibial plateau fractures with three incisions combined approach is effective,which can shorten the recovery time of patients,improve the function of knee joint and reduce postoperative complications.
