龙虎交战针法针刺力敏腧穴治疗气滞血瘀型腰椎间盘突出症的效果

摘　　要：目的:总结力敏腧穴分布规律,探讨力敏腧穴予龙虎交战针法对气滞血瘀型腰椎间盘突出症(LDH)患者的疗效。方法:选取2020年1月—2021年12月江西中医药大学附属医院针灸科住院及门诊96例气滞血瘀型腰椎间盘突出症患者作为研究对象,根据治疗方式不同将其分为力敏针刺组(龙虎交战针法针刺力敏腧穴治疗)及传统针刺组(龙虎交战针法针刺传统腧穴治疗),各48例。总结敏化腧穴分布规律,比较两组临床疗效、治疗前后改良Oswestry功能障碍指数(ODI),视觉模拟评分法(VAS)评分、血清白细胞介素-6(IL-6)、前列腺素E2(PGE2)指标。结果:首次探查气滞血瘀型LDH患者,前10名敏化腧穴分别为:委中、天枢穴区、曲泉、肾俞、阳陵泉、承山、内关、风市穴区、秩边、膈俞。治疗2个疗程后,力敏针刺组总有效率为93.75%、愈显率为75.00%,高于传统针刺组的85.42%和62.50%,但两组组间对比差异无统计学意义(P>0.05);两组VAS和改良ODI评分均较治疗前明显下降,力敏针刺组的下降程度较传统针刺组更为显著(P<0.05);两组血清IL-6、PGE2指标均较治疗前下降,力敏针刺组的下降程度较传统针刺组更为显著(P<0.05)。结论:气滞血瘀型LDH患者大部分力敏腧穴主要分布在足三阳经、手足厥阴经中,予龙虎交战针法治疗,LDH气滞血瘀证的效果显著,可明显减轻LDH患者的疼痛,改善腰椎各项功能,使血清IL-6、PGE2指标下降可能是其起效的作用机制之一。Objective:To summarize the distribution rule of force-sensitive acupoints and explore the therapeutic effects of dragon-tiger fighting acupuncture at force-sensitive dragon-tiger fighting acupuncture at force-sensitive on lumbar disc herniation(LDH)of Qi stagnation and blood stasis type.Method:A total of 96 inpatients and outpatients with LDH of Qi stagnation and blood stasis type in the Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine from January 2020 to December 2021 were selected as the research objects.According to the different treatment methods,they were divided into the force-sensitive acupuncture group(the dragon-tiger fighting needling method was used to treat force-sensitive acupoints)and the traditional acupuncture group(the dragon-tiger fighting needling method was used to treat traditional acupoints),with 48 cases in each group.The distribution of sensitized acupoints was summarized,and the clinical efficacy,modified Oswestry disability index(ODI),visual analogue scale(VAS)score,serum interleukin-6(IL-6)and prostaglandin E2(PGE2)before and after treatment were compared between the two groups.Result:The top 10 sensitized acupoints were Weizhong,Tianshu acupoint,Ququan,Shenshu,Yanglingquan,Chengshan,Neiguan,Fengshi acupoint,Zhibian and Geshu in the first exploration of LDH patients with Qi stagnation and blood stasis type.After 2 courses of treatment,the total effective rate was 93.8%and the cure rate was 75.0%in the force-sensitive acupuncture group,which were higher than 85.4%and 62.5%in the traditional acupuncture group,but there was no significant difference between the two groups(P>0.05);the VAS and modified ODI scores in the two groups were significantly lower than those before treatment,and the decrease in the force-sensitive acupuncture group was more significant than that in the traditional acupuncture group(P<0.05);the serum levels of IL-6 and PGE2 in the two groups were lower than those before treatment,and the reduction degree in the force-sensitive acupuncture gro
