摘 要:目的研究热性惊厥(Febrile seizures,FS)对学龄期儿童智力、注意力及品行方面的远期影响。方法选取40例单纯性热性惊厥(Simple febrile seizures,SFS)儿童作为SFS组、38例复杂性热性惊厥(Complex febrile seizures,CFS)儿童作为CFS组,以上78例儿童测试时均距首次诊断≥5年,同时设置正常家庭健康儿童40例作为对照组。受试者完成智商测试、视听觉注意力持续操作测验(Integrated visual and auditory continuous performance test,IVA-CPT),其父母完成注意力相关、品行相关问卷评估,分析对比数据。结果三组儿童智商无明显差别。IVA-CPT结果显示CFS组的综合控制力商数、综合注意力商数、听觉注意力商数、视觉注意力商数、多动商数及持续性商数均小于SFS组及对照组,CFS组的视觉控制力商数小于对照组,差异有统计学意义(P<0.05)。IVA-CPT中注意缺陷多动障碍(Attention deficit hyperactivity disorder,ADHD)、SNAP-Ⅳ评定量表中注意力不集中和对立违抗、Conners父母症状问卷(Parenting symptoms questionnaire,PSQ)中冲动-多动及Rutter行为父母问卷结果显示三组间异常率比较,差异有统计学意义(P<0.05)。结论FS对儿童智力无明显影响;SFS对儿童注意力、品行方面无明显影响;CFS会对学龄期儿童注意力、品行方面造成损害,可能是造成ADHD、品行障碍的原因之一;CFS儿童视听觉注意力商数、视觉控制力商数均较低,注意力不集中现象比起多动更为显著,因此进行ADHD相关治疗时,可以优先对这些方面进行训练。Objective To study the long-term effect of febrile seizures(FS)on intelligence,attention and conduct of school-age children.Methods Forty children with simple febrile seizures(SFS group)and 38 children with complex febrile seizures(CFS group)were selected,all 78 children were tested≥5 years after their first diagnosis,and 40 controls were also set up(control group).The subjects completed the IQ test,the integrated visual and auditory continuous performance test(IVA-CPT),and their parents completed the attention-related and conductrelated questionnaires evaluation then analyzed and compared the data.Results There was no significant difference in IQ among the three groups.The composite control quotient,composite attention quotient,auditory attention quotient,visual attention quotient,hyperactivity quotient and persistence quotient in the CFS group were lower than those in the other two groups,and the visual control quotient in the CFS group was lower than that in the control group,with statistically significant differences(P<0.05).There were statistically significant ratios among the three groups for the results of attention deficit hyperactivity disorder(ADHD)in the IVA-CPT,inattention and oppositional defiance in the SNAP-Ⅳscale,and impulsivity-hyperactivity in the Conners parenting symptoms questionnaire and Rutter behavior parenting questionnaire(P<0.05).Conclusion FS has no significant effect on children's intelligence.SFS has no significant effect on children's attention and conduct.CFS can cause damage to school-age children's attention and conduct,and may be one of the causes of ADHD and conduct disorders.Children with CFS have poor audiovisual attention quotient and visual control quotient,and inattention is more pronounced than hyperactivity.Training in these areas can be prioritized in the treatment of attention deficit hyperactivity disorder-related treatment.
