摘  要:目的探讨冠状动脉慢性完全闭塞病变(CTO)患者血清内脏脂肪素水平、25-羟基维生素D3[25-(OH)D3]与冠状动脉侧支循环(CCC)的关系。方法共纳入189例经冠状动脉造影证实为CTO的患者;根据Rentrop Cohen分级将189例患者分为CCC建立不良组(n=82,Rentrop 0,1级)和CCC建立良好组(n=107,Rentrop 2,3级)。ELISA法检测血清内脏脂肪素和25-(OH)D3水平。结果CCC建立不良组内脏脂肪素水平明显高于CCC建立良好组(P<0.001),25-(OH)D3水平明显低于CCC建立良好组(P<0.001)。Spearman相关性分析显示,Rentrop分级与内脏脂肪素水平呈负相关(r=-0.692,P<0.001),与25-(OH)D3水平呈正相关(r=0.689,P<0.001)。Logistic回归分析显示内脏脂肪素、25-(OH)D3水平、LDL-C与糖尿病是CCC建立不良的独立危险因素。结论血清内脏脂肪素、25-(OH)D3、LDL-C水平与糖尿病对CCC建立情况影响较大,对评估CCC建立情况具有重要的临床价值。Objective To explore the relationship between serum visfatin level,25-hydroxy-vitamin D3[25-(OH)D3]and coronary collateral circulation(CCC)in patients with chronic total coronary occlusion.Methods A total of 189 patients with CTO confirmed by coronary angiography were included.189 patients were divided into poor collateral circulation formation group(n=82,Rentrop 0,1 grade)and good collateral circulation formation group(n=107,Rentrop 2,3 grade)according to the Rentrop-Cohen grade.ELISA method was applied to measure serum visfatin and 25-(OH)D3 levels.Results Visfatin level in the poor collateral circulation formation group was significantly higher than that in the good collateral circulation formation group(P<0.001),and 25-(OH)D3 level was significantly lower than that in the good collateral circulation formation group(P<0.001).Spearman correlation analysis showed that Rentrop grade was negatively correlated with visfatin level(R=-0.692,P<0.001)and positively correlated with 25-(OH)D3 level(R=0.689,P<0.001).Logistic regression analysis showed that visfatin,25-(OH)D3 levels,LDL-C and diabetes were independent risk factors for poor collateral circulation formation.Conclusion The level of serum visfatin,the level of 25-(OH)D3,LDL-C level and diabetes are related to the formation of CCC.
