Analysis on first CD4^(+)T lymphocytes test of newly reported HIV/AIDS cases in Zibo city,Shandong province from 2018 to 2022

摘  要:目的了解2018-2022年山东省淄博市新报告HIV/AIDS病例的首次CD4^(+)T淋巴细胞(简称CD4细胞)检测情况,评估免疫状况,分析影响因素,为制定针对性防控措施提供依据。方法从中国疾病预防控制信息系统艾滋病综合防治信息系统获得2018-2022年山东省淄博市新报告HIV/AIDS病例信息,比较新报告病例数、首次CD4细胞检测率、检测及时性及首次CD4细胞计数情况及年度变化趋势,分析首次CD4细胞检测病例CD4细胞计数、首次检测及时性的影响因素。结果2018-2022年山东省淄博市新报告HIV/AIDS病例678例,病例数逐年下降(曲线回归F=40.641,P=0.008)。完成首次CD4细胞检测577例(85.10%),各年度检测率不同(χ^(2)=17.615,P=0.001),但年度变化趋势差异无统计学意义(χ_(趋势)^(2)=3.170,P=0.075)。首次检测CD4细胞计数<200/mm3的169例,占29.29%,年度差异无统计学意义(χ^(2)=3.308,P=0.508),年度变化趋势差异无统计学意义(χ_(趋势)^(2)=2.166,P=0.141)。首次CD4细胞检测及时519例,及时率89.95%,年度检测及时性差异有统计学意义(χ^(2)=10.751,P=0.030),但年度变化趋势差异无统计学意义(χ_(趋势)^(2)=1.858,P=0.173)。影响因素分析结果显示,年龄(χ^(2)=8.327,P=0.016)和样本来源(χ^(2)=10.113,P=0.039)为病例首次CD4细胞检测计数是否小于200/mm3的影响因素。结论2018-2022年山东省淄博市新报告HIV/AIDS病例数逐渐下降,首次CD4细胞检测比例和及时性均无明显上升趋势,近1/3病例发现时已处于重度免疫缺陷状态。Objective To understand the first detection of CD4^(+)T lymphocytes(CD4 cells)in newly reported HIV/AIDS cases in Zibo city from 2018 to 2022,evaluate the immune status and analyze the influencing factors,so as to provide basis for formulating targeted prevention and control measures.Methods The information of newly reported HIV/AIDS cases in Zibo city from 2018 to 2022 was obtained from the Comprehensive AIDS Prevention and Control Information System.The number of newly reported cases,the first CD4 cell detection rate,detection timeliness,and the first CD4 cell count,as well as the annual trend of changes were compared.And the influence factors of CD4 cell count and the timeliness of initial testing were analyzed.Results In the past 5 years,678 new HIV/AIDS cases have been reported in Zibo city,with the number of cases decreasing year by year(F=40.641,P=0.008).A total of 577 cases(85.10%)completed the first CD4 cell test,with different annual detection rates(χ^(2)=17.615,P=0.001),but the annual change trend was not significant(χ^(2)_(trend)=3.170,P=0.075).There were 169 cases with CD4 cell count<200/mm^(3) detected for the first time,accounting for 29.29%.The annual difference was not statistically significant(χ^(2)=3.308,P=0.508),and there was no significant annual change trend(χ_(trend)^(2)=2.166,P=0.141).The first CD4 cell detection was performed in 519 cases in a timely manner,with a timely rate of 89.95%.There was a statistical difference in the timeliness of annual detection(χ^(2)=10.751,P=0.030),but there was no significant annual change trend(χ_(trend)^(2)=1.858,P=0.173).The results of influencing factor analysis showed that gender,age,occupation,marital status,educational level,history of sexually transmitted diseases,infection routes,and sample source had no statistically significant impact on the timeliness of CD4 cell detection in a case.Age(χ^(2)=8.327,P=0.016)and sample source(χ^(2)=10.113,P=0.039)were the influencing factors for whether the CD4 cell detection count for the first time in a
