S-100吸收性止血绫在1例冷凝血患者术中的应用

摘　　要：报告1例冷球蛋白血症患者在术中应用S-100吸收性止血缓的临床效果。患者为1例44岁卵巢癌女性,行卵巢癌根治术后发生腹部转移。患者入院后行血常规等化验,结果显示患者为冷球蛋白血症。兰州大学第二医院多学科会诊讨论后,结合患者手术意愿,为其施行脾切除术及肝转移灶切除术。术中,除常规电刀止血外,于手术创面覆盖S-100吸收性止血绫进行止血。术毕,待冲洗腹腔干净后,针对巨大创面的渗血,采用常规的电凝止血等措施,并针对患者冷球蛋白血症的特点,再将S-100吸收性止血缓2片溶于冰凉的生理盐水中,形成溶胶状后、将其均匀涂于创面,凝血效果明显。出血是外科手术中无法回避的临床问题,此文通过报告兰州大学第二医院1例冷球蛋白血症患者在术中应用S-100吸收性止血绫的情况,证实除了超声刀、电刀等常规外科止血措施之外,在术中联合应用新型可吸收凝血材料S-100,临床效果更为理想。Report the clinical effect of using S-100 absorbable hemostatic gauze during surgery in a patient with cryoglobulinemia.The patient is a 44 years old female with ovarian cancer who developed abdominal metastasis after undergoing radical ovarian cancer surgery.After admission,blood routine tests and other tests were conducted,and the results showed that the patient had cryoglobulinemia.After MDT discussion in our hospital,combined with the patient's surgical willingness,splenectomy and liver metastasis resection were performed on them.During surgery,in addition to conventional electric knife hemostasis,S-100 absorbable hemostatic gauze is covered on the surgical wound for hemostasis.After the surgery is completed and the abdominal cavity is washed clean,in addition to conventional electrocoagulation and hemostasis measures,in order to address the characteristics of the patient's cryoglobulinemia,two pieces of S-100 absorbable hemostatic gauze are dissolved in cold physiological saline to form a gel,and the latter is evenly applied to the wound,resulting in significant coagulation effects.Bleeding is an unavoidable clinical issue in surgical procedures.This article reports the application of S-100 absorbable hemostatic gauze during surgery in a patient with cryoglobulinemia in our hospital,confirming that in addition to conventional surgical hemostatic measures such as ultrasound knife and electric knife,the combined use of a new absorbable coagulation material S-100 during surgery has a more ideal clinical effect.
