糖尿病伴抑郁中医药研究进展

摘　　要：糖尿病是当前比较常见的一种慢性病。根据国际糖尿病联合会的数据显示,目前,我国糖尿病患病人口居世界首位,同时糖尿病易出现并发症,累及眼、脑、心、肾等脏器,由此发生糖尿病性视网膜病变、糖尿病肾病、心脑血管及其他有关的疾病,所以,对糖尿病的诊断和治疗已经不可忽视。随着生物-心理-社会-医学模式的发展,以及糖尿病的发生率、增长率逐年提高,人们开始密切注意情绪状态在糖尿病患者生理和心理方面的作用。抑郁指情绪低落、伴有焦虑等的精神障碍性疾病类型,常伴有睡眠障碍、头痛、慢性疼痛及其他症状。当患者处于抑郁状态下,会出现思维、意志、认知等方面的功能减退。研究表明,情绪低落会影响糖尿病患者的胰岛细胞功能水平,加重胰岛素抵抗。故糖尿病患者伴抑郁应在早期及时诊断并给予干预治疗,关注他们的心理健康状况,对患者血糖控制及生活质量等均有很大提高。文章通过对近年糖尿病伴抑郁相关文献的查阅,总结糖尿病伴抑郁的病因病机,对糖尿病伴抑郁中医药治疗的发展现状进行归纳整理,为今后临床治疗糖尿病伴抑郁提供方法和思路,进一步提高中医药治疗糖尿病伴抑郁的临床疗效。Diabetes is one of the most common chronic diseases,and according to the data of the International Diabetes Federation,at present,our country is the first in the world in terms of the number of people suffering from diabetes.At the same time,diabetes is prone to complications,involving the eyes,brain,heart,kidneys and other organs,resulting in diabetic retinopathy,diabetic nephropathy,cardiovascular and other related diseases,therefore,the diagnosis and treatment of diabetes can no longer be ignored.With the development of the bio-psycho-social-medical model,and the incidence and growth rate of diabetes increasing year by year,people have begun to pay close attention to the role of emotional state in the physiological and psychological aspects of diabetic patients.Depression is a type of mental disorder with depressed mood and concomitant anxiety as the main clinical manifestations,and can often be accompanied by sleep disorders,headaches,chronic pain and other symptoms.When patients are in a depressed state,they will experience reduced thinking,will and cognitive functions.Studies have shown that depressed mood can affect the level of islet cell function in diabetic patients and aggravate insulin resistance.Therefore,early diagnosis and intervention in diabetic patients with depression and attention to their mental health status can greatly improve their glycemic control and quality of life.According to the literature review of diabetes mellitus complicated with depression syndrome in recent years,etiology and pathogenesis of diabetes mellitus,and the development of TCM in the treatment of diabetic nephropathy are summarized.It will provide the methods and ideas for clinical research to improve the clinical effect of TCM on diabetes mellitus.
