孕晚期母胎依恋影响因素分析

摘　　要：目的:调查孕晚期孕妇母胎依恋情况并分析相关影响因素,探讨其与产前抑郁关系。方法:方便抽样法,于2023年1-4月选取219例孕晚期孕妇为研究对象,通过自行设计的包含基线资料、产前依恋问卷(PAI)和爱丁堡产后抑郁量表(EPDS)的调查问卷进行问卷调查。结果:219例PAI得分67.24±6.09分,条目分3.20±0.69分;产前EPDS得分6.38±1.53分,产前抑郁发生率11.9%。Pearson相关性分析显示,孕妇PAI得分与产前EPDS得分呈负相关(P<0.05)。多元回归分析显示,孕妇年龄高、家庭人均月收入高、初产妇、不良孕产史、母乳喂养、产前EPDS评分低均是影响孕晚期孕妇PAI得分的主要影响因素(P<0.05)。结论:孕晚期孕妇母胎依恋情况受年龄、经济状况以及心理因素等多重影响,可针对性提出干预措施,积极降低孕妇抑郁情绪,改善母胎依恋关系,促进母婴健康。Objective:To investigate the situation of the maternal-fetal attaching during the third trimester of pregnancy,and to study the correlation between the maternal-fetal attaching and the maternal prenatal depression.Methods:219 pregnant women during the third trimester of pregnancy were selected as study objects by convenience sampling method from January to April 2023.A self-designed survey questionnaire was used to investigate,including the baseline data and the scores of prenatal attachment questionnaire(PAI) and Edinburgh Postpartum Depression Scale(EPDS).Results:The PAI scores of the 219 women were 67.2 points and 46.09 points,and the items score of the women were 3.20 points and 0.69 points.The prenatal EPDS score of the women was 6.38±1.53 points and the incidence of prenatal depression of the women was 11.9%.Pearson correlation analysis showed that PAI score of the pregnant women was negatively correlated with their prenatal EPDS score(P<0.05).Multiple regression analysis showed that the advanced age,the high family per capita monthly income,the primipara,the adverse maternal history,the breastfeeding,and the low prenatal EPDS score of the women were the main factors influencing their PAI score during the third trimester of pregnancy(P<0.05).Conclusion:The maternal-fetal attachment status of the pregnant women during the third trimester of pregnancy is influenced by multiple factors,such as age,economic status,and psychological factors.Targeted intervention measures can actively conducted to reduce the maternal depression,and can improve their maternal-fetal attaching relationship,thereby promoting the maternal and infant health.
