针药治疗青少年干眼症临床辨证体会

摘　　要：干眼症又称角结膜干燥症、干燥性角膜结膜炎,是指多种原因引起的泪液质量异常,或动力学异常导致的泪膜稳定性下降。患者多伴有眼部不适和(或)眼表组织损害,出现干涩感、异物感、畏光、视物模糊和视疲劳等症状。近年青少年干眼症发病较多,病程长,目前眼科临床多采用人工泪液治疗,但治标不治本,容易复发。中医特色的辨证论治和经络学说在治疗干眼症中拥有良好的疗效。天水市中医医院辛瑞副主任长期临床经验总结青少年干眼症与电子视频终端的普及、长时间近距离用眼增多和屈光不正有关。在治疗上,他将干眼症分为脾胃虚弱、肺脾湿热、肝经郁热、气阴两虚四型辨证论治。同时将中医辨证分型与西医临床分型相对应,根据不同分型采用不同的辅助治疗,取得了良好效果。文章通过案例,介绍辛瑞副主任中医辨证分型治疗干眼症经验,以期为临床提供参考。Xerophthalmia also known as keratoconjunctivitis sicca,refers to the abnormal tear quality caused by lots of reason,or the decline of tear film stability caused by abnormal dynamics.Most patients are accompanied by eye discomfort or ocular surface tissue damage,and symptoms such as dryness,foreign body sensation,photophobia,blurred vision and visual fatigue.In recent years,adolescent xerophthalmia has a high incidence and a long course.At present,artificial tears are often used in ophthalmology clinic,but the symptoms are not cured and it is easy to relapse.TCM syndrome differentiation and meridian theory can produce good effects on xerophthalmia.XIN Rui,deputy director of Tianshui Hospital of Traditional Chinese Medicine,has concluded from a long-term clinical summary that adolescent dry eye is related to the popularity of electronic video terminals,long-term close eye use and ametropia.In terms of treatment,he divided dry eyes into four types of syndrome differentiation:spleen and stomach weakness,lung and spleen damp heat,liver meridian stagnation heat,and Qi(气)and Yin(阴)deficiency.He corresponded the syndrome differentiation type of TCM to the clinical type of western medicine,and adopted different auxiliary treatments according to different types,which achieved good results.This article introduces XIN Rui’s experience in treating xerophthalmia based on syndrome differentiation and classification through cases,with a view to providing reference for clinical practice.
