苏稼夫教授“以通为用”理论治疗中风后肢体功能障碍临证经验

摘　　要：中风是指以猝然昏倒,不省人事,半身不遂,口眼喎斜,语言不利为主症的病证。其致残率极高,常遗留有吞咽困难、抑郁、肢体功能障碍等各类后遗症,肢体功能障碍是中风后最常见后遗症之一,其主要临床表现为患侧肢体的感觉、运动的异常。本病多由内伤积损,又叠加劳逸失度、情志不遂、饮酒饱食等因素触发。其病位在心脑,与肝肾密切相关。全国第四批名老中医药专家苏稼夫教授认为,中风病后肢体功能障碍的基本病机为气虚血瘀,脏腑气机不畅,经络运行不通,恢复气血经络的通畅是治疗此病的关键。临床上,苏教授谨察病机,“以通为用”,针药并行,取穴精准,施术合法,治疗中风病后肢体功能障碍取得了显著疗效。文章结合案例介绍苏稼夫教授“以通为用”理论辨治中风后肢体功能障碍的经验,以期为了临床提供参考。Stroke refers to the syndrome of sudden fainting,unconscious,hemiplegia,facial paralysis and unfavorable language.Its disability rate is very high,often leaving behind various sequelae such as dysphagia,depression and limb dysfunction.Limb dysfunction is one of the most common sequelae after stroke,and its main clinical manifestation is the abnormal sensation and movement of the affected limb.The disease is mostly caused by internal damage,and superimposed overwork,emotional failure and other factors trigger.The disease is located in the heart and brain,which is closely related to liver and kidney.Professor SU Jiafu,the fourth group of famous Chinese medicine experts in China,believes that the basic pathogenesis of limb function after stroke is Qi(气)deficiency and blood stasis,the internal organs and Qi mechanisms are not smooth,and the meridians are not working properly.The key to treating this disease is to restore the smoothness of the Qi and meridians.In clinical practice,Professor SU has achieved remarkable results in the treatment of limb dysfunction after stroke by selecting accurate acupoints and using reasonable treatment methods.This article introduces Professor SU Jiafu’s experience in treating limb dysfunction after stroke based on the theory of taking unblocked transmission as normal functional state in combination with cases,with a view to providing reference for clinical practice.
