新九针疗法为主治疗膝关节骨性关节炎临证体会

摘　　要：膝关节骨性关节炎是临床上中老年人常见疾病。该病一般临床表现为膝关节肿胀疼痛、酸软、僵硬、行走困难等,严重者可导致患者残废,生活不能自理。在临床中治疗本病主要方法为口服药物、关节腔注射药物等,药物以非甾体抗炎药、镇痛剂、阿片类药物为主,不良反应较多,手术疗法创伤大且治疗成本高。针灸治疗膝关节骨性关节炎有明显的优势。山西省针灸研究所曹玉霞副主任医师擅长运用新九针治疗各种骨关节疾病。她强调整体与局部结合、治标与治本并重,分别运用火针、毫针、锋钩针、小针刀这四种针具,治疗膝关节骨性关节炎,并取得了良好效果。文章探讨膝关节骨性关节炎中医病机以及新九针疗法,介绍新九针疗法为主治疗膝关节骨性关节炎的具体操作方法,并分析每种针具的作用机制。期待通过总结新九针为主综合治疗膝关节骨性关节炎的特色诊疗方案,为中西医治疗膝关节骨性关节炎提供新的思路与方法。Knee osteoarthritis is a common disease of middle-aged and elderly people clinically.The clinical manifestations of the disease are swelling,pain,stiffness,etc.,and even disability,resulting in inability to take care of oneself.In the clinic,the main methods of treatment of this disease are oral drugs,intra-articular injection drugs,etc.,and the drugs are mainly nonsteroidal anti-inflammatory drugs,analgesics,opioids,with many adverse reactions,large trauma and high treatment cost of surgical therapy.Acupuncture has obvious advantages in treating knee osteoarthritis.CAO Yuxia,deputy chief physician of Shanxi Acupuncture Research Institute,is good at treating various bone and joint diseases with new nine-needles.In the process of treating knee osteoarthritis,she emphasizes the combination of whole and part,treating both symptoms and root causes,respectively using fire needle,filiform needle,front crochet needle and small needle knife,which achieved significant efficacy.This paper discusses the pathogenesis of knee osteoarthritis in TCM and the new nine-needle therapy,introduces the specific operation method of the new nine-needle therapy for knee osteoarthritis,and analyzes the mechanism of each needle.This article looks forward to providing new ideas and methods for the treatment of knee osteoarthritis by summing up the characteristics of the new nine-needles.
