王济梅运用厚朴麻黄汤经验

摘　　要：厚朴麻黄汤出自张机(字仲景)《金匮要略》,主治饮热郁肺证,在呼吸系统疾病中应用较多。厚朴麻黄汤与小青龙汤加石膏汤、射干麻黄汤、越婢加半夏汤有相似之处,但在药物组成、病机侧重等方面明显不同。在厚朴麻黄汤临床运用中,需精准辨证。只要辨证得当,灵活运用,厚朴麻黄汤可广泛应用于各种内科疾病的治疗中,为临床提供更多选择。太原市中医医院王济梅主任从医30余年,对呼吸系统疾病有自己独特的见解,临床擅于运用经方治疗各种内科疾病,谨守病机,辨证准确,施治得当,取得显著疗效。王济梅主任强调在临证应用中需抓病机、抓主证,如核心病机为饮热郁肺,主证则表现为脉浮、咳喘、饮证、汗出等方面,治以化饮除热、宣肺化痰。文章通过对厚朴麻黄汤原文的分析、类方的鉴别,以及典型案例分析,介绍王济梅主任运用厚朴麻黄汤治疗内科疾病经验,以期为临床提供参考。The Houpo Mahuang decoction(厚朴麻黄汤)comes from the medical sage ZHANG Ji’s Jingui Yaolue(《金匮要略》),is mainly used to treat a variety of diseases of phlegm and heat stagnation in lung.It is widely used in respiratory diseases.The Houpo Mahuang decoction has some similarities with the Xiaoqinglong decoction(小青龙汤)plus the Shigao decoction(石膏汤),the Shegan Mahuang decoction(射干麻黄汤),the Yuebi Banxia decoction(越婢加半夏汤),but there are obvious differences in medicine composition and pathogenesis focus.In clinical application of the Houpo Mahuang decoction,precise syndrome differentiation is needed.Based on proper differentiation,the Houpo Mahuang decoction can be flexibly and widely used in the treatment of various internal diseases,providing more clinical options.Director WANG Jimei from Taiyuan Hospital of Traditional Chinese Medicine,has been a doctor for more than 30 years.Director WANG has her own unique insights on respiratory diseases.She is good at using classical prescriptions to treat various internal diseases in clinical practice.She strictly abides by the pathogenesis,accurately distinguishes symptoms and signs,treats diseases properly,and obtains remarkable curative effects.Director WANG Jimei emphasizes that in the clinical application of the Houpo Mahuang decoction,it is basic to obtain the pathogenesis and the main syndrome.For example,when the core pathogenesis is phlegm and heat stagnation in lung,the main syndromes are manifested as floating pulse,cough and asthma,dampness syndrome,sweating,and so on.The treatment is to dispel dampness and remove heat,and ventilate lung and dissipate phlegm.In this article,Director WANG’s experience in treating internal diseases with the Houpo Mahuang decoction is introduced through the analysis of the original text of the Houpu Mahuang decoction,the identification of similar prescriptions,and typical cases,with a view to providing more reference for clinical practice.
