从瘀热论治小儿急乳蛾

摘　　要：急乳蛾指以发热,喉核急发红肿疼痛,状如乳蛾或蚕蛾为主要表现的咽喉疾病,此病主要指西医学中的急性扁桃体炎。小儿急乳蛾为西医中的小儿急性扁桃体炎、小儿急性化脓性扁桃体炎。当今社会发展迅速,儿童的生活环境、饮食结构、情志等发生变化,导致该病致病因素更为复杂,并且该病发病率逐年增高,以四岁以上小儿较为多见,逐渐受到家长的重视。瘀热论是国医大师周仲英首次提出的理论,认为热郁结于气分热不是瘀热,而血瘀化热也只是疾病的某一种病理现象,两种说法在面对临床重症辨证论治方面不具有指导意义,故而提出了瘀热相搏证。周老认为部分疾病发展到一定程度会出现火热邪毒或兼夹痰湿壅于血分的病机,血热相互博结为瘀从而形成血瘀、血热,两种病理因素相互结合、博结的一种证型。该理论多用于治疗各种内科杂病,且临床取得较好疗效,但极少运用于儿科疾病的治疗。文章认为小儿急乳蛾的病情发展到一定程度可用瘀热论协助治疗,在常规治法的基础上加以清热活血药物治疗部分小儿急乳蛾取得较好的疗效。文章结合案例,介绍从瘀热论治小儿急乳娥,以期为临床提供参考。通过本次研究,临床发现运用清凉药品之余用少许凉血活血化瘀药具有良好的治疗效果,通过瘀热论治疗小儿部分疾病值得今后临床上进一步研究运用。Acute tonsillitis refers to a throat disease characterized by fever,swelling and pain in the throat nucleus,and its shape is like a silkworm moth.This disease mainly refers to the acute tonsillitis in western medicine.Western medicine for acute tonsillitis is also known as pediatric acute tonsillitis and pediatric acute purulent tonsillitis.Today’s rapid development of society,children’s living environment,diet structure,emotions,etc.have changed,resulting in more complex pathogenic factors of the disease,and the incidence of the disease is increasing year by year,more common in children over four years old,and gradually attracted the attention of parents.The theory of stasis is the theory first proposed by the master of traditional Chinese medicine,ZHOU Zhongying,who believes that heat and depression in Qi(气)heat are not stasis,and blood stasis fever is only a pathological phenomenon of the disease,and the two statements are not of guiding significance in the face of clinical severe disease differentiation and treatment,so the proof of stasis and heat is proposed.TCM master ZHOU believes that some diseases develop to a certain extent,there will be hot evil poison or phlegm wet in the blood,blood heat into each other into stasis,thus forming blood stasis,blood heat,two pathological factors combined with each other,a type of syndrome.This theory has been applied in clinical treatment of various internal diseases and achieved good curative effect.However,this theory is rarely used in the treatment of pediatric diseases.Through clinical follow-up,the author believes that acute tonsillitis can be treated with stasis-heat theory when the disease develops to a certain extent.On the basis of conventional therapy,the treatment of some acute tonsillitis with heat-clearing and blood-activating medicines has achieved good curative effects.Combined with cases,this article introduces the treatment of children acute tonsillitis from the perspective of blood stasis and heat,with a view to providing reference for clinical
