摘  要:目的探讨布地奈德联合沙丁胺醇雾化吸入对小儿支气管哮喘气道重塑、血清高迁移率族蛋白B1(HMGB1)、25羟维生素D[25(OH)D]水平的影响。方法将144例支气管哮喘患儿根据治疗方案不同分为对照组(72例,沙丁胺醇雾化吸入)和观察组(72例,在对照组基础上给予布地奈德雾化吸入)。比较两组的临床效果。结果观察组的治疗总有效率高于对照组,退热时间及咳嗽、肺部哮鸣音、气喘消失时间均短于对照组(P<0.05)。治疗后,观察组的用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、第1秒用力呼气容积占预计值百分比(FEV_(1)%)、最大呼气流速峰值(PEF)、25(OH)D水平均高于对照组,气管内壁面积(Wai)、气道平滑肌面积(Wam)、支气管总管壁面积(Wat)、转化生长因子β_(1)(TGF-β_(1))、骨桥蛋白(OPN)、基质金属蛋白酶9(MMP-9)、HMGB1水平均低于对照组(P<0.05)。结论布地奈德联合沙丁胺醇雾化吸入治疗小儿支气管哮喘能有效提高临床效果,改善肺通气功能,降低气道重塑,促进机体更快恢复。Objective To investigate the effects of budesonide combined with salbutamol aerosol inhalation on airway remodeling,serum high mobility group protein B1(HMGB1)and 25-hydroxy vitamin D[25(OH)D]levels in children with bronchial asthma.Methods A total of 144 children with bronchial asthma were divided into control group(72 case,salbutamol aerosol inhalation)and observation group(72 case,given budesonide aerosol inhalation on the basis of the control group)according to different treatment schemes.The clinical effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the antipyretic time and the disappearance time of cough,lung wheezing and asthma were shorter than those in the control group(P<0.05).After treatment,the forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1)),percentage of forced expiratory volume in one second as expected(FEV_(1)%),peak expiratory flow(PEF)and 25(OH)D level in the observation group were higher than those in the control group,while the airway inner wall area(Wai),airway smooth muscle area(Wam),total bronchial wall area(Wat),transforming growth factor-β_(1)(TGF-β_(1)),osteopontin(OPN),matrix metalloproteinase-9(MMP-9)and HMGB1 levels were lower than those in the control group(P<0.05).Conclusion Budesonide combined with salbutamol aerosol inhalation in the treatment of bronchial asthma in children can effectively improve the clinical effect,improve the pulmonary ventilation function,reduce airway remodeling,and promote faster recovery of the body.
