摘　　要：目的为社区家庭药师居家药学服务模式的建立提供参考。方法对社区1例伴多种慢性病老年患者开展居家药学服务,包括药物治疗方案的评估、处方精简、药物重整、用药指导、药学咨询、科普宣教、家庭药箱管理、健康行为干预等,并定期随访、评估。通过案例列举介绍社区家庭药师参与居家药学服务的内容和心得。结果家庭药师在社区居家药学服务中具有重要作用,个体化的居家药学服务、良好的沟通和人文关怀可有效解决居家慢性病患者长期用药的相关问题。结论开展社区居家药学服务有助于降低医疗成本,减少用药错误,提高用药观念,促进居家患者安全、合理用药。Objective To provide a reference for the establishment of a home-based pharmaceutical care mode of community family pharmacists.Methods An elderly patient with multiple chronic diseases in community was provided home-based pharmaceutical care(including evaluation of medication therapy plan,prescription simplification,medication restructuring,medication guidance,pharmaceutical consultation,propaganda and education of popular science,management of family medical kit,health behavior intervention),regular follow-up and evaluation was carried out.The content and experience of community family pharmacists participating in home-based pharmaceutical care was introduced through cases.Results Family pharmacists played an important role in the home-based pharmaceutical care in community.Individualized home-based pharmaceutical care,good communication and humanistic care could effectively solve the related problems of long-term medication for patients with chronic disease at home.Conclusion Home-based pharmaceutical care in community can reduce medical costs,medication errors,improve medication awareness,promote safe and rational drug use for patients at home.
