左西替利嗪胶囊应用于成人变应性鼻炎患者中的临床效果及其对细胞因子水平的影响

摘　　要：目的探讨成人变应性鼻炎患者采用左西替利嗪胶囊治疗的临床效果及其对细胞因子水平的影响。方法回顾性选取100例成人变应性鼻炎患者的临床资料,依据用药方法不同分为枸地氯雷他定单独用药组(单独用药组)和枸地氯雷他定联合左西替利嗪胶囊用药组(联合用药组),各50例。比较两组患者用药前后的鼻炎症状积分、生活质量、细胞因子水平及临床疗效。结果用药后,两组患者的鼻痒、鼻塞、咽痒、眼痒、喷嚏、流涕积分及总分均明显低于本组用药前,且联合用药组明显低于单独用药组,差异有统计学意义(P<0.05)。用药后,两组患者的鼻部症状、眼部症状、其他症状、情感反应、实际问题评分及总分均明显低于本组用药前,且联合用药组明显低于单独用药组,差异有统计学意义(P<0.05)。联合用药组治疗总有效率96.00%(48/50)高于单独用药组的80.00%(40/50),差异有统计学意义(P<0.05)。用药后,两组患者血清免疫球蛋白(Ig)E、肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-4、IL-5、IL-17水平均低于本组用药前,血清干扰素-γ(IFN-γ)、IL-2、IL-10水平均高于本组用药前,且联合用药组血清IgE、TNF-α、IL-4、IL-5、IL-17水平分别为(160.50±8.10)IU/ml、(8.51±1.34)ng/L、(46.10±5.93)ng/L、(20.12±3.30)ng/L、(32.45±4.28)ng/L,均低于单独用药组的(276.43±9.54)IU/ml、(10.67±1.81)ng/L、(61.37±7.37)ng/L、(43.18±7.28)ng/L、(37.78±4.61)ng/L,血清IFN-γ、IL-2、IL-10水平分别为(24.28±3.33)ng/L、(1.38±0.20)ng/L、(67.37±7.56)ng/L,均高于单独用药组的(21.67±3.27)ng/L、(0.86±0.16)ng/L、(61.82±7.53)ng/L,差异有统计学意义(P<0.05)。结论成人变应性鼻炎患者治疗中应用左西替利嗪胶囊临床效果好,能够有效改善患者的临床症状及细胞因子水平,提高生活质量。Objective To discuss the clinical effect of levocetirizine capsule in adult patients with allergic rhinitis and its influence on cytokine levels.Methods A total of 100 adult patients with allergic rhinitis were divided into single medication group and combined medication group according to different medication methods,with 50 cases in each group.The single medication group was treated with desloratadine citrate alone,and the combined medication group was treated with desloratadine citrate and levcetirizine capsule.The scores of rhinitis symptoms and quality of life,cytokine levels before and after medication,and clinical efficacy of the two groups were compared.Results After medication,the scores of nasal itching,nasal congestion,pharyngeal itching,eye itching,sneezing and runny nose and total score in both groups were significantly lower than those before medication in this group,and the combined medication group was significantly lower than the single medication group.The differences were statistically significant(P<0.05).After medication,the scores of nasal symptoms,eye symptoms,other symptoms,emotional reactions,practical problems and total score in both groups were significantly lower than those before medication in this group,and the combined medication group was significantly lower than the single medication group.The differences were statistically significant(P<0.05).The total effective rate of the combined medication group was 96.00%(48/50),which was higher than that of 80.00%(40/50)of the single medication group,and the difference was statistically significant(P<0.05).After medication,the serum levels of immunoglobulin(Ig)E,tumor necrosis factor-α(TNF-α),interleukin(IL-4),IL-5 and IL-17 in both groups were lower than those before medication in this group,and the serum levels of interferon-γ(IFN-γ),IL-2 and IL-10 were higher than those before medication in this group;the serum levels of IgE,TNF-α,IL-4,IL-5 and IL-17 in the combined medication group were(160.50±8.10)IU/ml,(8.51±1.34)ng/L,(46.10±5.
