痰热清注射液辅助治疗呼吸机相关性肺炎(痰热闭肺证)的临床疗效

摘　　要：目的观察痰热清注射液辅助治疗呼吸机相关性肺炎(VAP)(痰热闭肺证)的临床疗效。方法选取2021年1月-2022年8月珠海市中西医结合医院收治的78例VAP(痰热闭肺证)患者,按照计算机随机分组法分为纯西药组和中药辅助组,各39例。纯西药组予以抗生素等常规西药治疗,根据患者病情决定治疗时间;中药辅助组在纯西药组基础上予以痰热清注射液持续治疗2周。比较2组临床疗效,治疗前、治疗2周后T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+))、炎性指标[γ干扰素(IFN-γ)、白介素-6(IL-6)、降钙素原(PCT)]及抗生素使用时间。结果中药辅助组总有效率(87.18%)高于纯西药组(66.67%)(χ^(2)=4.622,P=0.032)。治疗2周后,2组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)高于治疗前,且中药辅助组高于纯西药组(P<0.05或P<0.01);2组血清IFN-γ、IL-6、PCT水平低于治疗前,且中药辅助组低于纯西药组(P<0.01)。中药辅助组抗生素使用时间为(9.32±2.65)d,短于纯西药组的(11.45±3.12)d(t=3.249,P=0.002)。结论痰热清注射液辅助治疗VAP(痰热闭肺证)患者的疗效确切,可有效减轻炎性反应,增强机体免疫功能,缩短抗生素使用时间。Objective To investigate the effect of Tanreqing injection in the adjuvant treatment of VAP(phlegm heat blocking lung syndrome).Methods A total of 78 cases of VAP(phlegm heat blocking lung syndrome)patients admitted to Zhuhai Hospital of Integrated Traditional Chinese and Western Medicine from January 2021 to August 2022 were selected,and they were divided into the western medicine group and the Chinses medicine auxiliary group by computer grouping method,39 cases in each group.Patients of the western medicine group were given conventional western medicine treatment such as antibiotic,the treatment time depended on the patients′condition and patients in the Chinses medicine auxiliary group were given Tanreqing injection based on the western medicine group for 2 weeks.Clinical efficacy,T lymphocyte subsets(CD3^(+),CD4^(+),CD4^(+)/CD8^(+)),inflammation factors(IFN-γ,IL-6,PCT)before and after 2 weeks of treatment,and antibiotic use time were compared between the two groups.Results The total effective rate of the Chinses medicine auxiliary group was 87.18%,which was higher than 66.67%of the western medicine group(χ^(2)=4.622,P=0.032).After 2 weeks of treatment,CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the two groups were higher than those before treatment,and the Chinses medicine auxiliary group were higher than those of the western medicine group(P<0.05 or P<0.01);Serum levels of IFN-γ,IL-6 and PCT in the two groups were lower than those before treatment,and the Chinses medicine auxiliary group were lower than those of the western medicine group(P<0.01).Antibiotic use time of the Chinses medicine auxiliary group was(9.32±2.65)d,which was shorter than(11.45±3.12)d in the western medicine group(t=3.249,P=0.002).Conclusion Tanreqing injection has an exact effect in the adjuvant treatment of VAP(phlegm heat blocking lung syndrome),it can effectively reduce inflammation,enhance immune function of body,and shorten antibiotic use time.
