鳖甲煎丸联合中药洗剂治疗寻常型银屑病的临床疗效

摘　　要：目的观察鳖甲煎丸联合中药洗剂治疗寻常型银屑病的临床疗效。方法选取2020年2月-2021年2月湖南中医药大学第二附属医院收治的寻常型银屑病患者100例,按照入院先后顺序分为对照组(n=50)和观察组(n=50)。对照组患者采用中药洗剂治疗,观察组患者在对照组基础上联合鳖甲煎丸治疗,2组患者均持续治疗3个月。比较2组临床疗效,治疗前及治疗1、2、3个月后皮损面积和严重程度指数(PASI)积分,治疗前及治疗3个月后血清白介素-17(IL-17)、白介素-31(IL-31)、肿瘤坏死因子-α(TNF-α)及一氧化氮合酶(NOS)、一氧化氮(NO)水平,不良反应。结果观察组治疗总有效率高于对照组(96.00%vs.84.00%,χ^(2)=4.000,P=0.046)。治疗1、2、3个月后,2组PASI积分均较治疗前降低,且观察组均低于对照组(P<0.01)。与治疗前比较,治疗3个月后2组血清IL-17、IL-31、TNF-α、NOS、NO水平均降低,且观察组低于对照组(P<0.01)。观察组不良反应总发生率低于对照组(6.00%vs.20.00%,χ^(2)=4.332,P=0.037)。结论采用鳖甲煎丸联合中药洗剂治疗寻常型银屑病的疗效确切,可有效改善患者皮损症状,减轻炎性反应,改善病情,且安全性较高。Objective To observe the effect of Biejiajian pills combined with traditional Chinese medicine lotion for psoriasis vulgaris.Methods One hundred patients with psoriasis vulgaris treated in the Second Affiliated Hospital of Hunan University of Traditional Chinese Medicine from February 2020 to February 2021 were selected,and they were divided into the control group(50 cases)and the observation group(50 cases)according to the order of admission.The control group was treated with traditional Chinese medicine lotion,and the observation group was treated with Biejiajian pills based on the control group,both groups were treated for 3 months.Clinical efficacy,PASI score before treatment and at 1,2,3 months after treatment,serum levels of IL-17,IL-31,TNF-α,NOS,NO before and after 3 months of treatment,and incidence of adverse reactions were compared between the two groups.Results The total effective rate in the observation group was significantly higher than that in the control group(96.00%vs.84.00%,χ^(2)=4.000,P=0.046).At 1,2,3 months after treatment,PASI score of the two groups was lower than that before treatment,and the observation group was lower than that in the control group(P<0.01).Compared with those before treatment,serum levels of IL-17,IL-31,TNF-α,NOS,NO after 3 months of treatment of the two groups were lower,and the observation group were lower than those of the control group(P<0.01).Incidence of adverse reactions in the observation group was significantly lower than that in the control group(6.00%vs.20.00%,χ^(2)=4.332,P=0.037).Conclusion Biejiajian pills combined with traditional Chinese medicine lotion have an exact clinical efficacy for psoriasis vulgaris,it can effectively improve the patients′lesions symptoms,reduce inflammatory response,improve the condition,and with higher safety.
