摘  要:目的 分析银川市新生儿先天性心脏病(CHD)信息化管理系统在新生儿CHD筛查项目管理中的应用及价值。方法 以银川市妇幼保健院2018年分娩的6 424例新生儿、2020年分娩的7 589例活产新生儿为研究对象,采用心脏杂音听诊结合经皮血氧饱和度测定的方法对研究对象进行CHD筛查,筛查阳性新生儿经心脏彩超检查确诊。2018年、2020年分别采用传统人工、“银川市先心一体化信息管理系统”进行CHD筛查项目管理,分析两种管理模式在CHD项目管理效率、筛查管理水平、高危孕妇管理、随访等方面的优劣。结果 2020年采用系统管理后新生儿CHD的筛查率(91.17%)、心脏彩超检测率(98.70%)、随访率(97.50%)均高于2018年人工管理的筛查率(79.92%)、心脏彩超检测率(88.17%)、随访率(80.70%),差异具有统计学意义(χ^(2)值分别为366.15、40.57、74.24,P<0.05),且在信息采集、结果录入、阳性召回、分级、转诊、随访等方面系统信息化管理优于人工管理;4 185例参与妊娠期高危因素调查的产妇中,具有高危因素的产妇分娩新生儿CHD确诊率高于非高危因素产妇,差异有统计学意义(χ^(2)=20.40,P<0.05),联合双指标筛查法后,CHD确诊率由4.80%上升至5.68%;凭借“银川市先心一体化信息管理系统”,共有15例CHD轻、中、重症患儿通过系统完成转诊手术治疗,随访预后良好。结论 利用信息化平台建立新生儿CHD管理模式有助于提高该病筛查管理效率及水平,值得进一步推广应用。Objective To investigate application of Neonatal Congenital Heart Disease(CHD)Information Management System of Yinchuan City in management of neonatal CHD screening project,and explore application value of the system in this region.Methods 6424 newborns who were born in 2018 and 7589 newborns who were born in 2020 in Yinchuan Municipal Maternal and Child Health Hospital were selected as study subjects.Heart murmur auscultation combined with percutaneous oxygen saturation measurement was used to screen CHD,and the positive newborns were confirmed by ultrasonic cardiography(UCG).In 2018 and 2020,the traditional manual management and Yinchuan CHD Integrated Information Management System were used to manage the CHD screening project,and the advantages and disadvantages of the two management modes in CHD project management efficiency,screening management level,high-risk pregnant women's management and follow-up were analyzed.Results In 2020,the screening rate of neonatal CHD(91.17%),UCG examination rate(98.70%)and follow-up rate(97.50%)after systematic management were significantly higher than the screening rate(79.92%),UCG examination rate(88.17%)and follow-up rate(80.70%)by manual management in 2018(x=366.15,40.57 and 74.24 respectively,all P<0.05).And in information collection,result entry,positive recall,grading,referral and follow-up,Yinchuan CHD Integrated Information Management System was better than the traditional manual management mode.Among 4185 pregnant women who participated in investigation of highrisk factor during pregnancy,the confirmed diagnosis rate of neonatal CHD in these pregnant women with high risk factors during pregnancy was higher than that in those pregnant women without high risk factors during pregnancy,and the difference was significant(χ^(2)=20.40,P<0.05).In addition,combined with dual indicator screening,the confirmed diagnosis rate of neonatal CHD increased from 4.80o%to 5.68%.Through the CHD Integrated Information Management System of Yinchuan City,a total of 15 children with mild,m
