中药热敷散局部热敷治疗老年膝骨性关节炎的临床效果

摘　　要：目的:分析中药热敷散局部热敷治疗老年膝骨性关节炎的临床效果。方法:选取2021年9月—2022年7月聊城市人民医院收治的老年膝骨性关节炎患者60例为观察对象,根据随机数字表法分为对照组与观察组,各30例。对照组给予单踝置换术治疗,观察组在对照组基础上给予中药热敷散局部热敷治疗。比较两组患者临床疗效、中医证候积分、疼痛情况、膝关节功能、疼痛介质、炎性因子。方法:观察组治疗总有效率高于对照组,差异有统计学意义(P=0.038);治疗后,观察组中医证候积分、疼痛评分低于对照组,膝关节功能评分高于对照组,差异有统计学意义(P<0.001);治疗后,观察组血清P物质、缓激肽、前列腺素E2水平低于对照组,差异有统计学意义(P<0.001);治疗后,观察组白细胞介素-6、基质金属蛋白酶-3、肿瘤坏死因子-α水平低于对照组,差异有统计学意义(P<0.05)。结论:中药热敷散局部热敷治疗老年膝骨性关节炎对其疼痛情况及膝关节功能改善作用显著,可减轻炎性反应,有助于控制病情。Objective:To analyze the clinical effect of localized hot compress of Chinese medicine Refusan in the treatment of osteo‐arthritis of the knee in the elderly.Methods:Sixty cases of elderly patients with osteoarthritis of the knee admitted to Liaocheng People's Hospital from September 2021 to July 2022 were selected for observation and divided into the control group and the obser‐vation group according to the method of randomized numerical table,each with thirty cases.The control group was treated with single ankle replacement,and the observation group was treated with localized hot compresses of Chinese medicine Refusan on the basis of the control group.Compare the clinical efficacy,Chinese medicine symptoms points,pain,knee joint function,pain mediators and in‐flammatory factors of the two groups.Results:The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P=0.038);After treatment,the observation group's TCM evidence points and pain scores were lower than those of the control group,and the knee function scores were higher than those of the control group,with statistically significant differences(P<0.001);After treatment,the levels of serum substance P,bradykinin and prostaglandin E2 in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.001);After treatment,the levels of interleukin-6,matrix metalloproteinase-3 and tumor necrosis factor-αin the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Chinese medicine Refusan lo‐calized hot compresses for the treatment of osteoarthritis of the knee in the elderly has a significant effect on the improvement of its pain condition and knee joint function,which can reduce the inflammatory reaction and help to control the condition.
