摘　　要：目的探讨老年重症卧床患者应用肠内营养支持联合针对性压疮预防护理的效果。方法选取2021年1月至2022年1月接收的老年重症卧床患者78例,按电脑数字表法随机分作2组,对照组39例予以肠内营养支持,观察组39例予以肠内营养支持联合针对性压疮预防护理,对比2组营养状态、免疫功能、压疮预防效果及生活质量。结果观察组干预后血清前白蛋白(PAB)、血清白蛋白(ALB)及转铁蛋白(TF)水平比对照组高(P<0.05);观察组干预后CD_(3)^(+)、CD_(4)^(+)水平比对照组高(P<0.05);观察组干预后压疮风险分级优于对照组(P<0.05),且观察组压疮发生率是5.13%,比对照组的20.51%低(P<0.05);观察组干预后健康调查简表(SF-36)各维度评分均比对照组高(P<0.05)。结论老年重症卧床患者应用肠内营养支持联合针对性压疮预防护理可有效改善其营养状态与免疫功能,降低压疮风险,减少压疮发生率,并进一步改善生活质量。Objective To investigate the application effect of enteral nutrition support combined with nursing care of targeted pressure ulcer prevention on elderly critically ill bedridden patients.Methods A total of 78 elderly critically ill bedridden patients were admitted to our hospital from January 2021 to January 2022.According to the computer digital table,they were randomly divided into control group and observation group,with 39 cases per group.All patients were given enteral nutrition support,and those in the observation group were additionally given nursing care of targeted pressure ulcer prevention.The nutritional status,immune function,pressure ulcer prevention effect and quality of life were compared between the two groups.Results In the observation group,serum prealbumin(PAB),serum albumin(ALB)and transferrin(TF)after intervention were significantly higher than those in the control group(P<0.05).CD3^(+)and CD4^(+)levels after intervention were significantly higher in the observation group than those in the control group(P<0.05).In the observation group,the risk classification of pressure ulcers after intervention was superior than that in the control group(P<0.05),and the incidence of pressure ulcers was significantly lower in the observation group than that in control group(5.13%vs 20.51%,P<0.05).In the observation group,the scores of each dimension in the 36-Item Short Form Survey(SF-36)after intervention were significantly higher than those in the control group(P<0.05).Conclusion The application of enteral nutrition support combined with nursing care of targeted pressure ulcer prevention in elderly critically ill bedridden patients can effectively improve their nutritional status,immune function and quality of life,as well as reduce the risk and incidence of pressure ulcers.
