摘　　要：目的探讨渐进性牵伸康复支具对手外伤术后指关节僵硬患者的应用效果。方法选取2019年7月至2020年10月于本院行手外伤术后指关节僵硬治疗的88例患者作为研究对象,随机分为对照组与观察组,每组44例。对照组采用常规康复治疗方法,观察组在对照组的基础上采用渐进性牵伸康复支具进行治疗,比较两组治疗前后指关节功能、临床疗效、治疗前后疼痛程度及治疗满意度。结果治疗前,两组远端指关节、近端指关节及掌指关节活动度比较差异无统计学意义;治疗后,两组远端指关节、近端指关节及掌指关节活动度均大于治疗前,且观察组大于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率为97.73%,高于对照组的79.55%,差异有统计学意义(P<0.05)。治疗前,两组视觉模拟评分法(VAS)评分比较差异无统计学意义;治疗后,两组VAS评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组治疗满意度高于对照组,差异有统计学意义(P<0.05)。结论渐进性牵伸康复支具治疗手外伤术后指关节僵硬患者效果显著,可促进患者指关节活动功能恢复,降低患者疼痛感,值得临床推广应用。Objective To investigate the application effect of progressive extension rehabilitation brace on patients with knuckle stiffness after hand trauma.Methods 88 patients with knuckle stiffness after hand trauma admitted to our hospital from July 2019 to October 2020 were selected as the study subjects,and they were randomly divided into the control group and the observation group,with 44 cases in each group.The control group was treated with conventional rehabilitation treatment,and the observation group was treated with progressive extension rehabilitation brace on the basis of the control group,the knuckle function before and after treatment,clinical efficacy,pain degree before and after treatment and treatment satisfaction were compared between the two groups.Results Before treatment,there were no significant differences in the range of motion of distal,proximal and metacarpophalangeal joints between the two groups;after treatment,the range of motion of distal,proximal and metacarpophalangeal joints of the two groups were higher than before treatment,and the observation group was higher than the control group,and the differences were statistically significant(P<0.05).The total effective rate in the observation group was 97.73%,which was higher than 79.55%in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in visual analogue scale(VAS)score between the two groups;after treatment,the VAS score of the two groups were lower than before treatment,and the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The treatment satisfaction in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion Progressive extension rehabilitation brace is effective in the treatment of patients with knuckle stiffness after hand trauma,which can promote the recovery of knuckle movement function and reduce the pain of patients,and
