摘　　要：目的:分析24h动态心电图联合血清视黄醇结合蛋白4(RBP4)、心型脂肪酸结合蛋白(H-FABP)在冠心病心肌缺血中的应用价值。方法:选取2019年11月~2020年11月本院收治的出现疑似冠心病并发心肌缺血患者90例为研究对象,将确诊为冠心病并发心肌缺血患者56例作为阳性组,冠心病未并发心肌缺血患者34例为阴性组。结果:阳性组RBP4、H-FABP水平高于阴性组,阳性组中急性心肌梗死患者RBP4、H-FABP水平较高(P<0.05)。阴性组、阳性组两组及阳性组中稳定性心绞痛患者、不稳定性心绞痛患者、急性心肌梗死患者心肌缺血ST低段压发作时间分布对比(P>0.05)。三项联合对冠心病心肌缺血的诊断价值较高(P<0.05)。结论:24h动态心电图联合血清RBP4、H-FABP对冠心病心肌缺血具有较高的诊断价值。Objective:To analyze the application value of 24h dynamic electrocardiogram combined with serum retinol binding protein 4(RBP4)and cardiac fatty acid binding protein(H-FABP)in myocardial ischemia of coronary heart disease.Methods:90 patients with suspected coronary heart disease complicated with myocardial ischemia admitted to our hospital from November 2019 to November 2020 were selected as the study objects,56 patients with confirmed coronary heart disease complicated with myocardial ischemia were selected as the positive group,and 34 patients with coronary heart disease without myocardial ischemia were selected as the negative group.Results:The levels of RBP4 and H-FABP in positive group were higher than those in negative group,and the levels of RBP4 and H-FABP in patients with acute myocardial infarction in positive group were higher(P<0.05).The time distribution of ST low segment pressure in patients with stable angina pectoris,unstable angina pectoris and acute myocardial infarction in negative group,positive group and positive group were compared(P>0.05).The value of the combination of three factors in the diagnosis of myocardial ischemia in coronary heart disease was higher(P<0.05).Conclusion:24h holter electrocardiogram combined with serum RBP4 and H-FABP has a high diagnostic value for myocardial ischemia in coronary heart disease.
