甲亢症及桥本氏甲状腺炎鉴别诊断中应用彩色多普勒超声的临床意义

摘　　要：目的:研究分析甲亢症及桥本氏甲状腺炎鉴别诊断中应用彩色多普勒超声的临床意义。方法:选取2022年1月~2023年1月本院收治的28例甲亢症患者及34例桥本氏甲状腺炎患者作为研究对象,在入院后均需要采取彩色多普勒超声检查。对比两组的超声检查结果。结果:甲亢症组的甲状腺内部血流、肿大及颈血管鞘旁淋巴结肿大超声表现、RI数值与桥本氏甲状腺炎组对比均未存在明显差异(P>0.05);而颈部Ⅳ、Ⅵ区淋巴结肿大、合并结节和有内部网状回声等超声表现、EDV、PSV和HR数值比较则存在明显差异(P<0.05);甲状腺实质内血流信号分布0级、Ⅰ级甲亢症组低于桥本甲状腺炎组,Ⅱ级、Ⅲ级高于桥本甲状腺炎组(P<0.05)。结论:在甲亢症及桥本氏甲状腺炎的鉴别诊断中,采用彩色多普勒超声具有良好的应用优势。Objective:To study and analyze the clinical significance of color Doppler ultrasound in the differential diagnosis of hyperthyroidism and Hashimoto’s thyroiditis.Methods:28 patients with hyperthyroidism and 34 patients with Hashimoto’s thyroiditis admitted to our hospital from January 2022 to January 2023 were selected as the study subjects.Color Doppler ultrasound examination was required after admission.Compare the ultrasound examination results between the two groups.Results:There were no significant differences in the ultrasound manifestations,RI values,and internal blood flow of the thyroid gland,enlargement,and neck vessel sheath lymph node enlargement between the hyperthyroidism group and the Hashimoto’s thyroiditis group(P>0.05);However,there were significant differences in ultrasound manifestations,EDV,PSV,and HR values compared to lymph node enlargement,combined nodules,and internal reticular echoes in the IV and VI regions of the neck(P<0.05);The distribution of blood flow signals in the thyroid parenchyma of grade 0 and grade I hyperthyroidism group was lower than that of Hashimoto’s thyroiditis group,and grades II and III were higher than that of Hashimoto’s thyroiditis group(P<0.05).Conclusion:Color Doppler ultrasound has good application advantages in the differential diagnosis of hyperthyroidism and Hashimoto’s thyroiditis.
