摘  要:目的 探讨沙格列汀联合二甲双胍在2型糖尿病患者中的临床效果及对血清同型半胱氨酸(Hcy)水平的影响。方法 选取60例2型糖尿病患者作为研究对象,按照随机数字表法分为对照组和观察组,各30例。对照组给予二甲双胍口服治疗,观察组给予二甲双胍联合沙格列汀口服治疗。比较两组治疗效果,治疗前后血糖及血清Hcy水平,不良反应发生情况。结果 观察组治疗总有效率为96.67%,高于对照组的73.33%,差异有统计学意义(P<0.05)。治疗24周后,观察组空腹血糖(FBG)、餐后2 h血糖(2 h PBG)、糖化血红蛋白(HbA1c)水平分别为(6.10±1.24)mmol/L、(6.38±1.19)mmol/L、(7.21±1.45)%,均低于对照组的(6.96±1.34)mmol/L、(7.64±1.23)mmol/L、(9.34±1.32)%,差异有统计学意义(P<0.05)。治疗8、16、24周后,观察组血清Hcy水平分别为(14.63±2.14)、(10.32±1.95)、(7.23±0.84)μmol/L,均低于对照组的(19.57±2.31)、(15.39±2.02)、(12.31±0.95)μmol/L,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 沙格列汀联合二甲双胍治疗2型糖尿病患者相比较传统单用药物更有利于改善患者血糖情况,同时能有效降低血清Hcy水平,未增加不良反应发生率。Objective To discuss the clinical effect of saxagliptin combined with metformin in patients with type 2 diabetes mellitus and its influence on serum homocysteine(Hcy).Methods A total of 60 patients with type 2 diabetes mellitus were studied and divided into control group and observation group according to random numerical table,with 30 cases in each group.The control group was treated with oral metformin,and the observation group was treated with oral metformin and saxagliptin.The therapeutic effect,blood glucose and serum Hcy levels before and after treatment,and the occurrence of adverse reactions were compared between the two groups.Results The total effective rate of treatment of the observation group was 96.67%,which was higher than that of 73.33%of the control group,and the difference was statistically significant(P<0.05).At 24 weeks after treatment,the levels of fasting blood glucose(FBG),2 h postprandial blood glucose(2 h PBG)and glycosylated hemoglobin(HbA1c)in the observation group were(6.10±1.24)mmol/L,(6.38±1.19)mmol/L and(7.21±1.45)%,which were lower than those of(6.96±1.34)mmol/L,(7.64±1.23)mmol/L and(9.34±1.32)%in the control group,and the differences were statistically significant(P<0.05).At 8,16 and 24 weeks after treatment,the serum Hcy levels in the observation group were(14.63±2.14),(10.32±1.95)and(7.23±0.84)μmol/L,which were lower than those of(19.57±2.31),(15.39±2.02)and(12.31±0.95)μmol/L in the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Saxagliptin combined with metformin in the treatment of type 2 diabetes mellitus patients can improve the blood glucose of patients compared with the traditional single drug regimen,and can effectively reduce the level of serum Hcy without increasing the incidence of adverse reactions.
