摘　　要：目的 探讨富血小板血浆(Platelet-rich plasma,PRP)联合皮瓣修复治疗糖尿病足溃疡创面的临床效果.方法 随机选取40例糖尿病足患者为试验组,40例糖尿病足患者为对照组;所有患者入院后进行2~3次清创、负压引流材料及骨水泥覆盖,依创面情况进行皮瓣移植手术;试验组使用PRP凝胶联合皮瓣移植进行治疗,对照组只采用皮瓣移植手术治疗;观察所有患者术后创口渗出情况、皮瓣愈合时间、术后7 d细菌培养阳性率、术后14 d炎症反应阳性率,比较两组患者的临床疗效.结果 试验组患者愈合时间比对照组明显缩短(P<0.05);试验组换药次数少于对照组,试验组纱布浸透层数少于对照组(P<0.05);试验组术后14 d炎症反应阳性率低于对照组,试验组术后细菌培养阳性率低于对照组(P<0.05).结论 利用PRP技术联合皮瓣修复糖尿病足溃疡创面临床效果显著.Objective To explore the clinical effect of platelet-rich plasma(PRP) combined with flap in the treatment of diabetic foot ulcer wounds.Method Forty patients with diabetic foot were randomly selected as the test group,and forty patients with diabetic foot were the control group.After admission,all patients were treated 2 to 3 times with debridement,covered with negative pressure drainage materials and bone cement,and carried out with flap transplantation according to the wound condition.The experimental group was treated with PRP gel combined with flap transplantation.The control group was only treated with flap transplantation;The number of layers soaked in gauze and the time of dressing changed,the healing time of the flap after the operation,the positive rate of bacterial culture 7 day after the surgery,the positive rate of inflammatory response 14 day after the surgery were observed,and the treatment effect of the two groups was compared.Results The healing time of patients in the test group was significantly shorter than that of the control group(P<0.05);The time of dressing change in the test group was less than that of the control group,and the number of layers soaked in gauze in the test group was less than that of the control group(P<0.05);The positive rate of inflammation 14 day after the surgery in the test group is lower than that of the control group,and the positive rate of bacterial culture 7 day after the surgery is lower than that of the control group(P<0.05).Conclusion The clinical effect of using PRP combined with the flap to repair diabetic foot ulcer wounds is remarkable.
