CT引导下经皮肺穿刺活检术在肺部占位性病变中的诊断价值分析

摘　　要：目的 分析在诊断肺部占位性病变中,CT引导下经皮穿刺活检术(CT-PTNB)的应用价值。方法 选取2021年3月至2023年3月期间在濮阳油田总医院收治的96例肺部占位性病变患者为研究对象,对患者均提供CT-PTNB,统计穿刺活检成功率、穿刺活检病理结果和并发症发生情况。结果 96例患者均穿刺成功,89例一次穿刺成功取出活检样本,7例2次穿刺成功取出活检样本;92例根据术后组织病理结果确诊,诊断率是95.83%(92/96);在明确诊断的92例患者中,63例确诊是恶性肿瘤,其中,肉瘤4例,小细胞癌18例,鳞癌12例,腺癌18例,腺磷癌11例;29例为良性病变,其中,慢性炎症14例,曲菌感染共4例,结核共5例,神经鞘瘤共6例;CT-PTNB术后患者并发症发生率是7.29%,其中,咯血3例,少量气胸2例,穿刺部位出血1例、胸膜反应1例,经治疗后并发症症状均全部消失。结论 肺部占位性病变中,CT-PTNB诊断价值高,并且操作简单、创伤性小,以及患者发生并发症率低,值得临床借鉴。Objective To analyze the application value of CT guided percutaneous needle biopsy(CT-PTNB)in the diagnosis of pulmonary space occupying lesions.Methods A total of 96 patients with pulmonary space occupying lesions admitted to Puyang Oilfield General Hospital from March 2021 to March 2023 were selected as the research subjects.CT-PTNB was provided to all 96 patients,and statistics were conducted on the success rate of puncture biopsy,pathological results of puncture biopsy,and incidence of complications.Results All 96 patients were successfully punctured,with 89 cases successfully removing biopsy samples in one puncture and 7 cases successfully removing biopsy samples in two punctures;92 cases were diagnosed based on postoperative histopathological results,with a diagnostic rate of 95.83%(92/96);Among the 92 patients with clear diagnosis,63 were diagnosed with malignant tumors,4 with sarcomas,18 with small cell carcinoma,18 with adenocarcinoma,12 with squamous cell carcinoma,and 11 with adenophosphorus carcinoma;29 cases were benign lesions,including 14 cases of chronic inflammation,4 cases of Aspergillus infection,5 cases of tuberculosis,and 6 cases of schwannoma;The incidence of postoperative complications in CT-PTNB patients is 7.29%,including 3 cases of hemoptysis,2 cases of mild pneumothorax,1 case of bleeding at the puncture site,and 1 case of pleural reaction.After treatment,all complications disappeared.Conclusion CT-PTNB has high diagnostic value,simple operation,minimal trauma,and low incidence of complications in pulmonary space occupying lesions,which is worthy of clinical reference.
