绝经期妇女取环困难应对策略的对照研究

摘　　要：目的 对取环困难的绝经期妇女给予不同的术前用药方案,探讨两种不同用药方案在绝经期妇女取环术中的应用效果。方法选取2020年1月至2022年12月期间在我院取环的112例绝经期妇女作为研究对象,遵循随机数字表法分组原则,将其分成观察组(n=56)与对照组(n=56)。两组均应用标准取环术,其中对照组在取环前连续服用7 d的戊酸雌二醇片,观察组取环前第1天给予口服米非司酮片,第2天术前2 h经阴道后穹隆部位加用米索前列醇片。对比两组在宫颈软化、取环效果以及围手术期指标情况。结果 观察组宫颈软化有效率高于对照组(P<0.05);观察组取环顺利率高于对照组(P<0.05);观察组的患者手术时间比对照组短,术中出血量比对照组少,术后疼痛程度评分比对照组低(P<0.05)。结论 对于绝经期取环困难妇女,在取环术前应用米非司酮联合米索前列醇的用药方案提前干预,能够取得良好宫颈软化效果,为患者顺利取环提供帮助,此外可减轻手术创伤以及助力术后早期康复。Objective To provide different preoperative medication regimens for menopausal women with difficulty in IUD removal,and to explore the application effects of two different medication regimens in IUD removal surgery for menopausal women.Methods 112 menopausal women who underwent IUD removal in our hospital from January 2020 to December 2022 were selected as the study subjects.Following the principle of random number table grouping,they were divided into an observation group(n=56)and a control group(n=56).Both groups underwent standard IUD removal surgery,with the control group receiving continuous administration of estradiol valerate tablets for 7 days before IUD removal,and the observation group receiving oral mifepristone tablets on the first day before IUD removal,Add misoprostol tablets to the posterior fornix area of the vagina 2 hours before surgery on the 2nd day.Compare the cervical softening,removal effect,and perioperative indicators between the two groups.Results The effective rate in the observation group was higher than that in the control group(P<0.05).The interest rate of taking ring in the observation group was higher than that in the control group(P<0.05);The patients in the observation group had shorter surgical time,less intraoperative bleeding,and lower postoperative pain scores compared to the control group(P<0.05).Conclusion For postmenopausal women with difficulty in IUD removal,early intervention with mifepristone combined with misoprostol before IUD removal can achieve good cervical softening effect,provide assistance for patients with smooth IUD removal,reduce surgical trauma,and assist in early postoperative recovery.
