督导式预警性护理联合早期被动训练在ICU患者机械通气治疗期间的应用研究

摘　　要：目的 探讨督导式预警性护理联合早期被动训练在ICU患者机械通气治疗期间的应用价值。方法 将我院在2021年8月至2022年8月期间接受ICU机械通气治疗的患者82例为观察对象,以随机数字表法分为观察组(n=41)、对照组(n=41),对照组给予常规护理,观察组给予督导式预警性护理联合早期被动训练,对比两组患者机械通气时间、ICU住院时间、呼吸机相关性肺炎(VAP)发生率、肌力评分以及护理满意度。结果 观察组机械通气时间、ICU住院时间短于对照组,住院费用低于对照组(P<0.05)。实施后1 d直至1周,观察组MRC评分高于对照组(P<0.05)。两组患者护理满意度对比差异显著(P<0.05)。观察组VAP发生率为7.32%(3/41),低于对照组的24.39%(10/41)(χ^(2)=4.479,P=0.034),观察组撤机成功率发生率为92.68%(38/41),高于对照组的73.17%(30/41),组间对比(χ^(2)=5.513,P=0.019)。结论 针对ICU患者机械通气治疗期间实施督导式预警性护理联合早期被动训练,在缩短患者机械通气以及ICU住院时间方面效果显著,能够有效降低患者VAP发生风险,提升患者肌力情况,缓和护患关系。Objective To explore the application value of supervised early warning nursing combined with early passive training in ICU patients during mechanical ventilation therapy.Methods A total of 82 patients who received mechanical ventilation therapy in ICU in our hospital from August 2021 to August 2022 were divided into observation group(n=41)and control group(n=41)by random number table method.The control group was given routine nursing,and the observation group was given supervised early warning nursing combined with early passive training.Mechanical ventilation time,ICU stay time,incidence of ventilators associated pneumonia(VAP),muscle strength score and nursing satisfaction were compared between the two groups.Results The mechanical ventilation time and ICU stay time of observation group were shorter than control group,and the hospitalization cost was lower than control group(P<0.05).From 1 day to 1 week after implementation,MRC score of observation group was higher than control group(P<0.05).There was significant difference in nursing satisfaction between the two groups(P<0.05).The incidence of VAP in the observation group was 7.32%(3/41),lower than that in the control group(24.39%,10/41)(χ^(2)=4.479,P=0.034).The incidence of success rate of withdrawal in the observation group was 92.68%(38/41),higher than that in the control group(73.17%,30/41).Comparison between groups(χ^(2)=5.513,P=0.019).Conclusions The implementation of supervised early warning nursing combined with early passive training during mechanical ventilation therapy in ICU patients has significant effects on shortening mechanical ventilation and ICU stay,which can effectively reduce the risk of VAP,improve the muscle strength of patients,and ease the relationship between nurses and patients.
