摘  要:尽管经皮冠状动脉介入治疗(PCI)技术不断完善进步,但冠状动脉严重钙化病变的治疗仍是挑战。通常预处理困难,术中并发症高,且术后长期预后欠佳。经皮冠状动脉腔内冲击波球囊导管成形术(IVL)作为一种新的冠状动脉钙化病变预处理技术,是利用声压力波有选择性地作用于冠状动脉内膜和中膜下的钙化病灶,在不损伤血管内膜完整性的前提下,使钙化松解并恢复血管的顺应性,引起国内医师广泛关注。然而应用中,需要规范操作,明确适应证,预防并发症的发生。因此,本文根据中国专家的临床实践和共识,遵循国家相关政策和法规,结合最新研究结果,制定专家建议,旨在引导和规范IVL技术在临床中的应用。Despite the continuous advancements in percutaneous coronary intervention(PCI)technology,the treatment of severe coronary artery calcification remains a significant challenge.Typically,it presents difficulties in pretreatment,resulting in a high incidence of clinical complications and unfavorable outcomes.Intravascular lithotripsy(IVL),as a novel technique for pretreating coronary artery calcifi cation,has garnered widespread attention among Chinese cardiologist.IVL employs selective acoustic pressure waves to target calcified lesions beneath the intima and media layers of the coronary artery,eff ectively disintegrating calcifi cations and restoring vessel compliance without compromising the integrity of the vascular endothelium.However,during its application,standardized procedures,precise indications,and the proactive prevention of complications are necessary.Therefore,this article,based on the clinical practice and consensus of Chinese experts,in accordance with relevant national policies and regulations,and integrating the latest research fi ndings,provides expert recommendations aimed at guiding and standardizing the clinical application of IVL technology.
