摘　　要：目的 研究常规生化检测项目EDTA-K_(3)抗凝血浆是否可以替代血清样本进行检测。方法 选取44例随机健康体检人群,同时抽取EDTA-K_(3)抗凝管和血清管并及时离心分离血浆及血清,在同一生化分析仪上,检测6项常规体检指标在EDTA-K_(3)抗凝血浆与血清中的差异。随机选取20例EDTA-K_(3)抗凝血浆于2h离心完毕上机检测,并放置4、6h再进行相同生化检测,对不同时间段的EDTA-K_(3)血浆结果进行比对分析。结果 EDTA-K_(3)抗凝血浆与血清在检测ALT、AST、UREA、Cr、TC、TG 6项生化指标时,差异有统计学意义(P<0.05),但r>0.75具有相关性。同时进行偏倚分析,6项指标均在允许范围内。绘制Bland-Altman图,评价2、4、6h不同放置时间的EDTA-K_(3)血浆样本结果的一致性情况,6h内EDTA-K_(3)血浆的检测结果之间的一致性显示,散点基本均落在95%一致性区间(即1.96个标准差范围内),说明一致性情况良好。结论 在检测ALT、AST、UREA、Cr、TC、TG 6项生化指标时,EDTA-K_(3)抗凝血浆可替代血清进行检测。6h内的EDTA-K_(3)抗凝血浆对检测结果无影响。Objective To investigate whether EDTA-K_(3) anticoagulant plasma can replace serum samples for the detection under special circumstances.Methods 44 random healthy cases were selected from physical examination population.EDTA-K_(3) anticoagulant tubes and serum tubes were extracted at the same time point.The plasma and serum were centrifuged,and the differences of six routine physical examination indexes in EDTA-K_(3) anticoagulant plasma and serum were detected with the same biochemical analyzer.Twenty cases of EDTA-K_(3) anticoagulant plasma for the same biochemical detection were randomly selected for machine detection after a 2-hour centrifugation,and placed for 4 hours and 6 hours.The results of EDTA-K_(3) plasma from different time durations were compared and analyzed.Results EDTA-K_(3) anticoagulant plasma and serum were used to detect the six biochemical indexes of ALT,AST,UREA,Cr,TC and TG.The results showed statistically significant differences(P<0.05),but r>0.75 was correlated.The biases analysis showed that all six indexes were within the allowable range.Bland-Altman plot was drawn to evaluate the consistency of EDTA-K_(3) plasma sample results at different placement time points of 2 hours,4 hours and 6 hours.The consistency of EDTA-K_(3) plasma test results within 6 hours showed that the scattered points basically fell within 95%consistency interval(i.e.,1.96 standard deviations).The consistency was acceptable.Conclusion EDTA-K_(3) anticoagulant plasma can replace serum in the detection of ALT,AST,UREA,Cr,TC and TG.EDTA-K_(3) anticoagulant plasma within 6 hours has no effect on the test results.
