摘　　要：目的:对比分析一次性筛查用支气管镜与普通支气管镜筛查早期中央型肺癌的可行性和有效性,探究一次性筛查用支气管镜的临床应用价值。方法:选取在医院就诊的500例中央型肺癌高危患者,按照检查方式的不同,将使用一次性筛查用支气管镜筛查的患者纳入观察组(170例),将使用普通支气管镜筛查的患者纳入对照组(330例),比较两组器械使用的有效率、安全性、阳性结果检出率及操作者临床满意度评分。结果:一次性筛查用支气管镜和普通支气管镜临床使用中均无出现器械故障,器械使用有效率均为100%。观察组一次性筛查用气管镜平均操作时间为2.16 min,与对照组平均操作时间5.68 min对比,观察组用时明显短于对照组,差异有统计学意义(t=42.743,P<0.001)。观察组的操作者临床操作满意度评分为4.08分,低于对照组的4.48分,两组比较差异有统计学意义(t=17.158,P<0.001)。结论:早期中央型肺癌一次性筛查用支气管镜在临床筛查中可行,并且具有筛查时间缩短、提高患者耐受性、避免重复消毒和操作简便等优势,可应用于临床筛查早期中央型肺癌。Objective:To compare and analyze the feasibilities and effectiveness of the disposable bronchoscope for screening and general bronchoscope in screening early-stage central lung cancer,and to explore the clinical application value of the disposable bronchoscope for screening.Methods:A total of 500 high-risk patients with central lung cancer in the hospital were selected.According to different examination methods,the patients who used disposable bronchoscope for screening were included in the observation group(170 cases),and the patients who used general bronchoscope for screening were included in the control group(330 cases).The effectiveness,the safety,the detection rate of positive results and the clinical satisfaction score of the operator between the two types of apparatuses in use were compared.Results:Both the disposable bronchoscope for screening and general bronchoscope did not occur failure,and the effective rates of the two apparatuses were 100%.The average operation time of the disposable bronchoscope for screening of the observation group was 2.16 minutes,which was significantly shorter than that(5.68 minutes)of the control group(t=42.743,P<0.001).The satisfaction score of the clinical operation of the operators in the observation group was 4.08 points,which was lower than 4.48 points in the control group,and the difference between the two groups was statistically significant(t=17.158,P<0.001).Conclusion:It is feasible that the disposable bronchoscope for screening early-stage central lung cancer,and it has a series of advantages include shorter screening time,improving the tolerance of patients,avoiding repetitive disinfection and convenient operation,which can be used in the clinical screening for early-stage central lung cancer.
