摘 要:目的:使用基于源的形态学测量(SBM)对创伤性脑损伤(TBI)患者恢复期默认模式网络(DMN)和执行控制网络(ECN)内脑区灰质(GM)体积结构异常变化进行研究。方法:选取OpenNeuro平台的TBI公共数据集中的临床资料,将数据集中的中重度TBI患者和年龄、性别与之匹配的健康对照(HC)受试者分别纳入TBI组(14例)和HC组(12名)。对两组受试者结构磁共振(sMRI)检查得到的T1结构像进行分割,使用约束SBM提取出DMN和ECN内的脑区灰质,对GM体积进行统计比较。结果:TBI组与HC相比,受伤后3个月(TBI-session1)时脑区GM体积在DMN内、左侧ECN内和右侧ECN内,均存在显著差异(t=-5.408,t=-2.569,t=-3.626;P<0.05);受伤后6个月(TBI-session2)时只有DMN内的GM与HC组有显著差异(t=-2.298,P<0.05);左侧ECN内和右侧ECN内脑区的GM体积均无显著差异。受伤后12个月(TBI-session3)时DMN内、左侧ECN内、右侧ECN内均无显著差异。DMN、左侧ECN和右侧ECN内均有部分脑区的GM体积在TBI后随时间发生显著变化,差异有统计学意义(F=9.369,F=6.383,F=5.537;P<0.05)。TBI后DMN和ECN内脑区GM体积的异常变化集中在楔前叶、扣带回、部分颞叶和部分额叶。结论:脑损伤会导致TBI患者恢复初期DMN和ECN内的GM体积显著萎缩,随时间推移灰质体积增加,中后期逐渐稳定,为TBI患者恢复期的认知功能障碍提供了依据。Objective:To study the abnormal changes of volume and structure of gray matter(GM)of brain area in the default mode network(DMN)and executive control network(ECN)of patients with traumatic brain injury(TBI)at convalescence by source-based morphometry(SBM).Methods:The TBI public database of OpenNeuro platform was selected,and 14 TBI patients was divided into TBI group,and 12 healthy subjects were selected as HC group at the same time.Both two groups underwent structural magnetic resonance imaging(sMRI)examination,and the obtained T1 structural images of two groups were further segment.The constrained SBM was used to extract the GM of brain area in DMN and ECN,and the GM volumes of two groups were compared by statistical method.Results:The differences of the GM volume of brain area of TBI-session1 in DMN,the ECN of the left side and the ECN of the right side between two groups were significant(t=-5.408,t=-2.569,t=-3.626,P<0.05),respectively.There was significant difference of the only GM in the DMN between two groups as TBI-session2(t=-2.298,P<0.05),and there were no significant differences in the GM volume of the brain area in the ECN of the left side and the ECN of the right side.There were no significant differences in the GM volume of the brain area in the DMN,the ECN of the left side and the ECN of the right side as TBI-session3.The GM volumes of partial brain areas in the DMN,the ECN of the left side and the ECN of the right side changed with the changing of time after TBI,the differences were significant(F=9.369,F=6.383,F=5.537,P<0.05),respectively.After TBI,the abnormal changes of GM volume of brain area in the DMN and the ECN concentrated at anterior cuneate lobe,cingulate gyrus,partial temporal lobe and partial frontal lobe.Conclusion:Brain injury can lead to significant atrophy of GM volume in the DMN and the ECN of TBI patients at the initial stage of convalescence,and the GM volume increased with the changes of time,which gradually stabilizes at the middle and later stage.It provides evidence for cogni
