彩色多普勒超声对股动脉穿刺术后假性动脉瘤诊疗中的影像学分析

摘　　要：目的:探讨彩色多普勒超声在股动脉穿刺术后假性动脉瘤(PSA)的诊疗价值。方法:选取医院收治的股动脉穿刺术后确诊并进行治疗的148例PSA患者,治疗前后超声随访PSA的内部回声、大小变化,瘤体内部血流变化情况等,分析超声诊断的准确率、漏诊率以及超声随访治疗后的效果。结果:148例患者中超声诊断股动脉穿刺术后PSA 139例,诊断准确率为93.9%(139/148),漏诊9例,漏诊率为6.1%(9/148)。148例患者中8例行手术治疗,未行手术治疗的140例患者治疗后3 d后彩色多普勒超声复查,其中125例患者包块明显缩小,15例患者包块缩小不明显;1个月后复查,130例患者包块完全消失,10例患者包块缩小至原体积的50%以下;3个月后复查,140例患者假性动脉瘤包块完全消失。结论:彩色多普勒超声对股动脉穿刺术后PSA具有很高的诊断率,是动态评估临床治疗效果的首选影像检查工具。Objective:To explore the value of diagnosis and treatment of color Doppler ultrasound for pseudoaneurysm(PSA)after femoral artery puncture.Methods:A total of 148 patients with PSA after femoral artery puncture who were confirmed in hospital were selected.Before and after treatment,the ultrasound was used to conduct follow-up for the changes of the internal echoes,the size and the internal blood flow within the PSA.And then,the accuracy,the rate of missed diagnosis and the treatment effect after follow-up with ultrasound were further analyzed.Results:In 148 patients,139 cases were confirmed as PSA after femoral artery puncture,and the accuracy rate of diagnosis was 93.9%(139/148),and missed diagnosis occurred in 9 cases,and the rate of missed diagnosis was 6.1%(9/148).In these patients,8 cases underwent surgical treatment,and the results of 140 cases without surgical treatment underwent color Doppler ultrasound reexamination after 3d indicated that the masses of 125 cases of them significantly reduced,and the reductions of the masses of 15 cases were not significant.The reexamination results after 1 month indicated the masses of 130 cases completely disappeared,and the masses of 10 cases had reduced to less than 50%of the original volume.The reexamination results after 3 months indicated that the pseudoaneurysm mass of 140 cases disappeared completely.Conclusion:Color Doppler ultrasound has higher diagnostic rate for the pseudoaneurysm after femoral artery puncture,which is the preferred imaging tool of dynamic assessing clinical treatment effect.
