摘 要:目的:分析颅脑外伤后患者诱发急性弥漫性脑肿胀的影响因素,探讨其临床结局,指导临床及时防治。方法:回顾性分析我院急诊科收治的297例颅脑外伤患者临床资料,根据患者颅脑外伤后有无急性弥漫性脑肿胀进行分组,其中57例存在急性弥漫性脑肿胀的患者纳入研究组,另240例无急性弥漫性脑肿胀患者纳入常规组。采用Logistic多因素分析法了解颅脑外伤后患者诱发急性弥漫性脑肿胀的影响因素。结果:Logistic分析结果提示患者年龄、受伤至入院时间≥3 h、低血压、原发性脑干损伤是颅脑外伤患者诱发急性弥漫性脑肿胀的高危因素(P<0.05)。研究组28 d死亡率显著高于常规组(P<0.05)。结论:颅脑外伤患者伴有低血压、原发性脑干损伤、受伤后至入院时间间隔≥3 h、青少年及儿童重型颅脑损伤等影响因素,诱发急性弥漫性脑肿胀的风险增高,需要临床及时治疗,以降低患者死亡率。Objective:To analyze the influence factors on the induction of acute diffuse brain swelling among patients with craniocerebral trauma,to discuss the clinical outcomes,and to guide timely clinical prevention and treatment.Methods:A retrospective analysis was carried out on the case data of 297 patients with craniocerebral trauma admitted to the Emergency Department of Yue Bei People's Hospital from January 2020 to March 2022,and the patients were divided into different groups according to whether they suffered from acute diffuse brain swelling after craniocerebral trauma.Among them,57 patients with acute diffuse brain swelling were included in the research group,and the remaining 240 patients without acute diffuse brain swelling were included in conventional group.Logistic analysis was used to analyze the influence factors on the induction of acute diffuse brain swelling among patients with craniocerebral trauma.Results:Logistic analysis results showed that the age of patients,time interval from injury to hospital admission≥3 h,low blood pressure,and primary brain stem injury were high-risk factors to induce acute diffuse brain swelling among patients with craniocerebral trauma(P<0.05).The 28 d death rate of the research group was significantly higher than that of the conventional group(P<0.05).Conclusion:For those patients with craniocerebral trauma suffering from low blood pressure and primary brain stem injury,or the time interval from injury to hospital admission is≥3 h,or children and young people suffering from severe craniocerebral injury,the risk of inducing acute diffuse brain swelling is higher,and timely clinical treatment is urgentto reduce the patients'death rate.
