摘  要:目的探讨多模态影像数据融合构建三维肿瘤模型在蝶骨嵴脑膜瘤手术中的应用。方法回顾性分析中国科学技术大学附属第一医院神经外科自2018年12月—2022年12月收治的82例蝶骨嵴脑膜瘤患者,利用术前头颅影像资料,通过3D-Slicer软件将多种序列影像数据配准融合,虚拟重建出蝶骨嵴脑膜瘤与大脑中动脉及周围重要神经等解剖结构的立体影像模型,比较术前是否建模对患者住院时间、总费用、感染、神经功能缺损、术中出血等方面的影响。结果与非建模组患者相比,术前建模的患者术中出血更少、总体感染率更低、住院时间更短、治疗总费用更低。结论基于多模态影像融合技术的术前三维建模可以为较复杂的蝶骨嵴脑膜瘤提供直观而准确的可视化信息,指导手术计划,有利于血管保护以及肿瘤精准切除。Objective To explore the application value of three-dimensional tumor model with multi-modal image fusion in the surgical treatment of patients with sphenoid ridge meningioma.Methods The clinical data of 82 patients admitted to Department of Neurosurgery,the First Affiliated Hospital of USTC from December 2018 to December 2022 with sphenoid ridge meningioma were analyzed retrospectively.Skull imaging were performed before the surgery,and 3D-Slicer multimodal image fusion technique was used to clarify the relationship between the tumor and the surrounding tissues including adjacent skulls,cranial nerves,important arteries and veins.The three-dimensional image models of sphenoid ridge meningioma,middle cerebral artery and peripheral important nerves were reconstructed.And the effects of preoperative modeling on the length of hospital stay,total cost,infection,neurological defects,intraoperative bleeding and so on were compared to the non-modeling group.Results Compared with patients in the non-modeling group,the preoperative modeled patients had less intraoperative bleeding,a lower overall infection rate,a shorter hospital stay,and a lower total cost of treatment.Conclusions Multimodal image fusion tumor model can provide a large amount of intuitive and accurate visual information for the complicated skull base tumor surgery,and effectively guide the surgery,so as to contribute to the protection of nerve function and accurate tumor resection.
