超声引导下穿刺硬化术与内镜下内引流术治疗肾盂旁囊肿的效果比较

摘　　要：目的比较超声引导下穿刺硬化术与内镜下内引流术治疗肾盂旁囊肿的临床效果。方法抽取2020年3月至2023年3月安阳市人民医院收治的78例肾盂旁囊肿患者,根据治疗方法分为对照组与观察组,每组39例。对照组给予输尿管软镜钬激光内切开引流术治疗,观察组给予超声引导下穿刺硬化术治疗。比较两组临床疗效、手术指标、氧化应激指标[去甲肾上腺素(NE)、肾上腺素(AD)、皮质醇(COR)]及并发症发生率。结果观察组临床总有效率(94.87%,37/39)高于对照组(71.79%,28/39),P<0.05。观察组手术时间、住院时间均短于对照组(P均<0.05),观察组术中失血量少于对照组(P<0.05)。治疗后,观察组血清NE、AD、COR水平均低于对照组(P均<0.05)。观察组并发症发生率(2.56%,1/39)低于对照组(15.38%,6/39),P<0.05。结论超声引导下穿刺硬化术治疗肾盂旁囊肿具有创伤性小、失血量少、恢复快等优点,可有效提高治疗效率,减轻应激反应,降低并发症发生率。Objective To compare the clinical efficacy of ultrasound-guided puncture sclerotherapy and endoscopic internal drainage in the treatment of parapelvic cyst.Methods A total of 78 patients with parapelvic cysts treated in Anyang People’s Hospital from March 2020 to March 2023 were selected,and they were divided into control group and observation group according to different treatment methods,with 39 cases in each group.The control group was given ureteral flexible holmium laser incision and drainage treatment,while the observation group was given ultrasound-guided puncture sclerotherapy.The clinical efficacy,surgical indexes,oxidative stress indexes assessed by norepinephrine(NE),epinephrine(AD)and cortisol and(COR),and the total incidence of complications were compared between the two groups.Results The total clinical effective rate of the observation group(94.87%,37/39)was higher than that of the control group(71.79%,28/39),P<0.05.The operative time and hospital stay in the observation group were shorter than those in the control group(all P<0.05),and the intraoperative blood loss in the observation group was lower than that in the control group(P<0.05).After treatment,the levels of serum NE,AD and COR in the observation group were lower than those in the control group(all P<0.05).The total incidence of complication of the observation group(2.56%,1/39)was lower than that of the control group(15.38%,6/39),P<0.05.Conclusions Ultrasound-guided puncture sclerotherapy has the advantages of less trauma,less blood loss and faster recovery in the treatment of parapelvic cyst,which can effectively improve the treatment efficiency,reduce the stress reaction and reduce the incidence of complications.
