糖尿病患者白内障术后后囊膜混浊的影响因素分析

摘　　要：目的探讨糖尿病患者白内障术后后囊膜混浊的影响因素。方法抽取2017年1月至2022年1月于济南市第二人民医院接受白内障手术治疗的糖尿病合并白内障患者150例,收集其临床资料,比较白内障术后后囊膜混浊出现者与未出现者的临床资料,将差异有统计学意义的资料代入Logistic回归方程,分析白内障术后出现后囊膜混浊的影响因素。结果白内障术后后囊膜混浊出现者(75例)与未出现者(75例)性别、年龄、眼别、术前眼轴比较差异未见统计学意义(P>0.05),两者糖尿病病程、术前糖化血红蛋白水平、是否合并糖尿病视网膜病变比较差异有统计学意义(P<0.05)。Logistic回归分析结果显示,糖尿病病程、术前糖化血红蛋白水平、是否合并糖尿病视网膜病变是糖尿病患者白内障术后出现后囊膜混浊的影响因素(P<0.05)。结论后囊膜混浊是糖尿病患者白内障术后多发的一种并发症,其影响因素有糖尿病病程、术前糖化血红蛋白水平、是否合并糖尿病视网膜病变。Objective To investigate the influencing factors of posterior capsular opacification after cataract surgery in patients with diabetes mellitus.Methods A total of 150 patients with diabetes mellitus complicated by cataract who underwent cataract surgery in Jinan Second People’s Hospital from January 2017 to January 2022 were selected.The clinical data of them were collected,and the clinical data were compared between patients with and without posterior capsular opacification after cataract surgery.The statistically significant data were substituted into the logistic regression equation to analyze the influencing factors of posterior capsular opacification after cataract surgery.Results There was no significant difference in gender,age,eye type,and preoperative axis oculi between patients(75 cases)with posterior capsular opacification and patients(75 cases)without the disease after cataract surgery(P>0.05).There were significant differences in the duration of diabetes mellitus,the level of preoperative glycosylated hemoglobin,and diabetic retinopathy between patients with and without posterior capsular opacification after cataract(P<0.05).Results of logistic regression analysis showed that the duration of diabetes mellitus,level of preoperative glycosylated hemoglobin,and diabetic retinopathy were the influencing factors of posterior capsular opacification in patients with diabetes mellitus after cataract surgery(P<0.05).Conclusions Posterior capsular opacification is a frequent complication after cataract surgery in diabetic patients,and its influencing factors include the duration of diabetes mellitus,preoperative glycosylated hemoglobin level,and diabetic retinopathy.
