异维A酸胶囊联合精准脉冲光治疗玫瑰痤疮的临床疗效

摘　　要：目的比较异维A酸联合精准脉冲光(Delicate Pulse Light,DPL)与异维A酸治疗玫瑰痤疮的临床效果及对炎性因子的影响。方法回分析顾性2018年6月—2021年6月我院100例玫瑰痤疮患者,按照治疗方式分为对照组(50例)和观察组(50例)。对照组给予异维A酸软胶囊,10mg/次,2次/d,共3个月;观察组给予DPL,4周一次,共3个月。观察临床疗效、皮损评分、痤疮特异性生活质量量表(Acne-QOL)、满意度及不良反应,酶联免疫法检测患者血清IL-1β、IL-1α、IL-8水平。结果观察组总有效率高于对照组(P=0.000)。治疗后,观察组丘疹脓疱、毛细血管扩张和鼻赘严重程度评分均低于对照组(P均=0.000)。治疗后,观察组患者血清IL-1β、IL-1α、IL-8水平均低于对照组(P均=0.000)。观察组治疗后疼痛程度评分高于对照组(P=0.000)。治疗后观察组Acne-QOL评分均高于对照组(P=0.001)。观察组患者不良反应发生率低于对照组(P=0.000)。结论异维A酸胶囊联合DPL治疗玫瑰痤疮疗效确切,能改善皮损症状,抑制炎症反应,不良反应少。Objectives To analyze the effect of isotretinoin with delicate pulse light(DPL)on rose acne and serum interleukin-1β(IL-1β)、interleukin-1α(IL-1α)and interleukin-8(IL-8).Methods 100 patients with rose acne in our hospital from January 2018 to January 2021 were selected and randomly divided into control group and observation group,50 cases in each group.Control group was treated with isotretinoin soft capsules,10 mg/d,twice daily.Observation group was treated with DPL and isotretinoin soft capsules.The treatment period was 3 months.The clinical efficacy,skin lesion score and adverse reactions were compared.Serum IL-1β、IL-1α、IL-8 levels were meared through ELISA.Results The total effective rate of observation group(88.0%)was higher than that of control group(66.0%)(χ2=6.438,P=0.000).After treatment,the pustules score,telangiectasia score and nasal polyps score were different between two groups(P all=0.000).After treatment,serum IL-1β、IL-1α、IL-8 levels were different between two groups(P all=0.000).The incidence of adverse reactions was different between two groups(χ2=30.879,P=0.000).Conclusion Isotretinoin capsule combined with DPL is effective in the treatment of rosacea,which can improve the skin lesion symptoms,inhibit the inflammatory reaction,have less adverse reactions.
