红斑狼疮斑片状秃发1例

摘　　要：目的了解红斑狼疮斑片状秃发的临床特点,以期提高该类疾病诊治水平。方法报告1例红斑狼疮相关的斑片状秃发患者的诊治情况以及患者临床、皮肤镜表现和头皮组织病理特点。结果红斑狼疮相关斑片状秃发临床表现为突然出现的、孤立的、边界清楚的红色或黄色秃发区。皮肤镜显示毛干变细,颜色变浅,头皮毛囊间区毛细血管扩张。头皮组织病理学显示基底细胞空泡化,血管附属器周围中等淋巴细胞浸润。治疗和原发疾病的控制后,毛发可恢复。结论红斑狼疮相关斑片状秃发临床表现易与斑秃混淆,但有独特的皮肤镜、病理改变,值得注意。Objectives To figure out the clinical features of patchy alopecia in lupus erythematosus in order to enhance the level of diagnosis and treatment of these diseases.Methods The diagnosis and treatment and the clinical,dermatoscopic and histopathological features of the scalp of a patient suffering from lupus erythematosus with patchy alopecia were reported.Results Lupus erythematosus with patchy alopecia is characterized by sudden,isolated,well-defined red oryellow alopecia. The dermatoscope showed that the hair stem became thinner,thecolor became lighter,and the interfollicular region of the scalp presented thetelangiectasia. Histopathology of the scalp showed vacuolization of basal cells andmoderate lymphocyte infiltration around vascular appendages. After reasonabletreatment for the disease,the hair condition gradually recovered. ConclusionLupus erythematosus with patchy alopecia has unique clinical manifestations andscalp changes.
