马拉色菌相关乳头乳晕角化过度症1例

摘　　要：患者女,22岁,双侧乳头、乳晕变黑、增生、斑块8余年。皮肤科情况:双侧乳头增大、肥厚、粗糙,表面呈黑褐色弥漫菜花样斑块,局部干裂,无红肿、溢液。实验室检查:HPV分型检测阴性。皮损组织病理示:乳头、乳晕角化过度伴角质层大量圆形、卵圆形孢子。PAS染色:角质层大量圆形、卵圆形孢子,部分出芽,未见菌丝。真菌镜检示:马拉色菌阳性。结合临床表现及辅助检测,诊断为马拉色菌相关乳头乳晕角化过度症。A 22-year-old female presented with darkened,hyperplasia and plaques on her nipples and areolas bilaterally for more than 8 years.Physical examination showed bilateral papillae were enlarged,thickened and rough,with dark brown diffuse cauliflower-like plaques on the surface,local dryness,and no redness,swelling or overflow were found.HPV typing test was negative.Histopathological examination showed hyperkeratosis with a large number of round and ovoid spores of the epidermis.PAS staining showed a large number of round and ovoid spores in the stratum corneum with some sprouting,no mycelium was found.Malassezia was found under fungal microscopy.The diagnosis of hyperkeratosis of nipple and areola with Malassezia was made in combination with clinical manifestations and ancillary tests.
