基于中西医病机融通从“痹”论治重症急性胰腺炎

摘　　要：重症急性胰腺炎(severe acute pancreatitis,SAP)是消化急危重症,病死率居高不下。中西医协同诊治SAP疗效优势明显,但中医治疗也遇到了诸多瓶颈。传统的辨证论治相对单一,难以全面触之SAP复杂病机;囿于“少阳阳明证”、“实热结胸证”的六经病机认识,用方相对局限,疗效难以突破;缺乏针对SAP不同病因、不同阶段以及不同并发症进行较为系统的中西医病机融通思考和探讨。谢胜教授带领脾胃治未病团队在消化重症SAP的中西医协同诊治方面积累了丰富临床经验。基于中西医病机融通深入思考,提出从“痹”论治重症急性胰腺炎,并针对“痹”之病机创立开宣解郁诸法,初步指导SAP临床诊治取得满意疗效。pancreatitis with severe acute pancreatitis(SAP)is characterized by acute digestive pancreatitis with a high morbidity rate.The synergistic diagnosis and treatment of SAP by traditional Chinese and western medicine has obvious curative effect advantages,but the treatment of traditional Chinese medicine has also encountered many bottlenecks.Traditional treatment based on syndrome differentiation is relatively simple and difficult to fully touch the complex pathogenesis of SAP.Limited by the understanding of the pathogenesis of the six Classics of"Shaoyangyang evident syndrome"and"solid heat chest knot syndrome",the prescription is relatively limited,and the curative effect is difficult to break through.There is a lack of systematic thinking and discussion on the integration of TCM and Western medicine with different etiology,stages and complications of SAP.Professor Xie Sheng led the spleen and stomach treatment team to accumulate rich clinical experience in the collaborative diagnosis and treatment of severe digestive SAP with traditional Chinese and Western medicine.Based on the in-depth thinking of integrated pathogenesis of traditional Chinese and Western medicine,this paper proposed to treat severe acute pancreatitis from the perspective of"impenetrable",and established the methods of promoting and relieving happiness for the impenetrable pathogenesis,preliminatively guiding the clinical diagnosis and treatment of SAP to achieve satisfactory efficacy.
