摘　　要：变应性接触性皮炎(ACD)是一种常见的炎症性皮肤病,属中医“漆疮”范畴。现代医学认为,影响ACD的发病因素包括遗传、环境及生活方式、特应性病史及皮肤屏障功能受损,其发病机制涉及机体免疫失调、炎症反应、氧化应激以及神经传导等。西医治疗ACD多采取糖皮质激素、类固醇及免疫抑制剂为主,还涉及热疗法、光疗法等。中医药治疗有口服给药、外用给药及针灸疗法等,能从免疫调节、抑制炎症反应、抗氧化、降低神经传导等多种途径达到治疗效果,且较西医治疗具有低复发率、高疗效的特点。论述ACD的发病机制、影响因素以及治疗方法,为防治ACD提供理论依据。Allergic contact dermatitis(ACD)is a common inflammatory skin disease,belonging to the category of“lacquer sore”in traditional Chinese medicine.Modern medicine believes that the pathogenesis of ACD includes genetic environment,lifestyle,history of atopic diseases,and impaired skin barrier function,and its pathogenesis involves immune disorder,inflammatory response,oxidative stress,and nerve conduction.Western medicine treats ACD mainly with glucocorticoids,steroids,and immunosuppressants,and also involves heat therapy and light therapy.TCM treatment includes oral administration,topical administration acupuncture,and moxibustion,etc.,which can achieve therapeutic effects from immune regulation,inhibition of inflammation,antioxidants,reduction of nerve conduction,and other ways,and has the characteristics of low recurrence rate and high curative effect compared with Western treatment.In summary,the pathogenesis factors and treatment methods of ACD were discussed to provide theoretical basis for the prevention and treatment of ACD.
